Question: Sir, i am 47 years old. i have history of digestion problems and having hard stools with occasional bleeding when i was 25-30 years and recently though subsided. some how i managed by having lot of water and sometimes with laxatives. for the last two years i have taken various medicines for occasional breathlessness and having pain in left shoulder and pulpitations. angio was done in 2013 diagnosed as mild desease in LAD.Iam on Ecospirin AV 75 for the last one year. Now since Aug14 i have taken antibiotics for skin infection and Typhoid fever (Turbocip for 21 days SUpracod200 for 4 days and Cepdoxime for 5 days since then again i have stomoch problem of hard stools with significant blood coming from Anus. Ihave stopped ecospirin for some days and again started taking.Now for the last 10 days i have complaint of Pulpitations, heavyness in chest, and shivering pains in left sideof the body very frequently with longer durations doctor has advised some blood tests in which RBS 122mg/dl BUrea 30mg/dl s.creatinine 1.1mg/dl, lipase 559U/L, Amylase190U/L. Ihave consulted cordiologist with the report he said nothing to worry as it is mostly not cardiac related and Further evaluation is needed when episode occurs. Upon Lipase and Amylase levels he expressed to consult gastroenterologist. Is there anything serious kindly advise

Hi.
Thanks for your query and an elucidate history.

You have problems related to hard stools with bleeding - raised Lipase and Amylase - normal sugar and kidney functions - breathlessness, shoulder pain left and palpitations for which you have consulted the cardiologist- reports noted- skin infection, Typhoid and the antibiotics, Ecosprin.

In such situations I would advise you the following:::
MRI and MRCP for the position of the liver, gall bladder and pancreas.
Repeat Lipase and Amylase. ( please post the reports to see the normal range)
Liver Function tests.
Bleeding Time, Clotting time, Prothrombin Time, Platelet count.
Urine test
Stool test.

You will also need to get Colonoscopy done to see for the reason of hard stool and bleeding.

If the Amylase and Lipase reports are above the normal range, there is a chance of Pancreatitis or pancreatic damage and you need to consult a Medical Gastroenterologist ASAP (as soon as possible). You may need to get admitted to have a proper diagnosis and treatment.

You can add probiotics, continue taking laxative in a dose suitable to have smooth bowel movements, take plenty of oral fluids to avoid dehydration.
