Welcome to HCM. I am Dr Anshul Varshney. I understand your concern.
Pioglitazone has been implicated and banned recently due to increase in bladder cancer and heart failure cases.
But got green flag again with warning status.
Means its up to you that you want to take it or not. Definitely there are chances of heart problem on use of this medication.
I personally use this medicine only in my patients who are not willing for insulin treatment and no other resort is left.

In such scenario I would advise to start on Metformin 1gm twice a day and Glimiperide 2 mg once a day.
I ask my patient to maintain record of self monitoring of blood sugar and then revert back after 10 days of charting.
You may observe this.
For high triglyc start exercises and take low fat diet.
Repeat Lipid profile after 2 months.
