Brief Answer:
Constant attention and support through care-taker necessary.

Detailed Answer:
Hi,
Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.

Good to know, her heart is holding up.

Based on your query, my opinion is as follows:

1. Disturbed sleep, poor nutrition and dehydration appears to be the causes.
2. Empty stomach causes gastritis. Also, poor sleep pattern increases stress on the body increasing acidity in the stomach. Hiatal hernia will further increase feeling of acidity along with rapid stomach filled feeling.
3. Dehydration along with hypoglycemia could be causing dizziness and exhaustion.
4. Poor appetite is secondary to weakness and disturbed sleep.

She will need to:
1. Have a regular sleep pattern of 8-10 hours. 8-9 hours in the night between 9-7 and daytime during afternoons, one hour of sleep.
2. Take multiple small meals. At least 6 times in a day. Avoid spicy food. It will reduce acidity and also control diabetes.
3. Good hydration. At least 5-6 liters of water per day on an average in normal conditions. If hot, increase by 1-2 liters more. Need good hydration.
4. If possible a caretaker to look into her nutrition, hydration, and medication.

As she is dizzy at times, ask her to move slowly to avoid any fall. Any fall or fractures at this age will be seriously detrimental and can affect her psychologically severely.

Hope it helps.
