Question : Hi, I was DX'ed with trigeminal neuralgia last year, and I have had chronic migraines nearly my whole life. Just a few days ago, I started having dizziness, and it makes my eyes not able to focus. These episodes are short & temporary, but it's miserable. I don't really get a migraine when they happen, but sometimes I do get a slight headache. I'm a bit scared, and not sure if I should call EMS or not.

I appreciate your concern

Dizziness with visual disturbances (especially if the onset is acute) should be dealt with in the ER, ideally. An assessment of vitals and general physical exam should be conducted. Metabolic profile and an EKG should also be advised. If these parameters are within normal range then the cause is likely to be attributed to the Gabapentin which you are using as it can cause dizziness and visual disturbances in up to 20 percent of patients taking it. Abrupt discontinuation of gabapentin should not be done especially if you have been taking it for more than 2 weeks.
