Suggest Treatment For Frequent Heavy Periods

Question: This is regarding my wife who has been having more frequent (20-21 days) and heavy periods over the past 2-3 months.

Recent USG TAS + TVS done on Day 15 of her cycle show the following: Bulky uterus 96x56x61mm. Endometrial thickness 12mm, homogenous endometrial echotexture. 1 Subcentimeter fibroid (6mm) is seen. Right ovary shows 2 functional cysts (20x16mm and 15x14mm). Prolactin levels within normal range. No other pain or anything out of ordinary.

A doctor suggested D&C but I've read that this is no longer the first choice of treatment. Please advise. Thanks.

It is true that a D&C is not usually the first line treatment, though it may help her in her case. The report unfortunately does not comment on the location of the fibroid. If the fibroid is "sumucosal" meaning that it is located in the lining of the uterus, then I would recommend that your doctor (or another doctor in your area who is capable of doing this) should hysteroscopically remove it (remove it directly by placing a small scope in the uterus). If you are no longer planning to have more children, she would also benefit by getting an ablation (a relatively easy procedure that burns the lining of the uterus. However, before doing any of this, she would need a biopsy to make sure there is no cancer in her uterus (This is rare, and I doubt that is the problem, but one needs to check first to make sure). A biopsy would also rule out infection which can cause similar problems as well.
