Suggest Treatment For Grade Two Acne Vulgaris

Question: Hi,
I am 23 years old and I have grade 2 pimple. I have consulted a dermatologist and she has prescribed me zinc tablets, retino ac 0.025, facewash containing glycolic acid. I Have enlarged pores on my skin( cheek, forehead,nose) She has adviced me to undergo micro needling rf treatment or crystal microdermabrasion to reduce it. I dont want to undergo invasive treatment as my pores are not much. I am planning to undergo micro dermabrasion. Will it be effective or medicines would only work. I am in big delima. Please suggest me what will be appropriate?

Hi, welcome to Healthcare Magic,

In grade two Acne Vulgaris, I used to prescribe Cap Minocycline 100 mg once daily.
Face wash containing 2% Salicylic acid is more effective in reducing excess keratinization.
Apply Retino AC 0.025% at night in small quantity as advised and Clindamycin cream at day time.
For enlarged Pores micro needling RF is more effective than crystal microdermabrasion.
Although Microdermabrasion, commonly known as skin polishing, is non invasive procedure and absolutely safe.
For better advice i need to see your clinical picture of your face.

I have uploaded images. Please hav a look. microdermaabrasion reduces pores to what extent? And how effective it will be on my skin?I have lots of black and whit heads as well a darker chin.. Will microdermaabrasion improves it? Shall I continue with same medicine or should do some changes. I hav heard about chemical peel also. Please suggest me as I m too much confused and I am facing acne problem since 10 yrs.

Hi, welcome again,
You have got active stage of acne, so postpone Microdermabrasion as bacterial infection of Propionibacterium acnes is there.
Wait till postular episodes to get over.
Microdermabrasion reduces pores 50-70%. It also improves darker chin.
Chemical peeling with salicylic acid 30% is good at this time, it not only helps in clearing acne & blackheads but also helps in reducing pore.
What about your menstrual cycle? is it regular? As polycystic ovarian disease is one of the commonest cause of recalcitrant acne.

Adding Minocycline orally and clindamycin topically at day time will help you. Use salicylic acid face wash.

Can u suggest me any good face wash? My cycles are regular but during periods pimple are more on my face especially chin and jawline area plus cheeks. Can I do peeling at home or should ask my doctor to do?. For microdermabrasion my doc has ask me to come after 15 days. And how many sittings will I require?.

I will suggest you to go for ultrasonography of lower abdomen to rule out polycystic ovarian disease.

No, chemical peelings are done under supervision of dermatologist as peeling is done with high concentration of acids and there are chances of burn.

Speelac face wash or Ahaglow S face wash can be used twice daily.
