Suggest Treatment For Hair Loss In Frontal And Mid Scalp Region

Question: Hi Doctor,



I am having hair loss which started around 2.5-3 years back. My hair line has not yet receded but I can notice reduced density especially around temples. Still people can't notice that I have hair loss. I see lot of hair fall during shower/ shampoo. On my mother's side there is no history of hair loss. My father had male pattern baldness. I was born quite late, my father as around 55 when I was born so by then he had full baldness ( hair only on back and sides). Looking at old pictures I think his hair loss started in mid thirties. Apart from this I smoke around 1 cigarette a day. One more thing is I often notice yellow oliy sticky fluid coming out from my scalp which forms hard lump. This fluid I also notice around back of ears. This fluid is especially noticeable if I have not shampooed for 1-2 days. I also use minoxidil foam since last 1 year. I have attached my latest WET hair picture. These are my questions:



1. Is this seborrheic dermatitis. If yes how to cure. I have just ordered Ketonext Coal Tar and Ketoconazole Shampoo to use twice/thrice weekly



2. Is my hair loss due to seborrheic dermatitis. If yes then will it stop/regrow after this is resolved.



3. Is my hair loss due to male pattern baldness inherited from my father. If yes I want to know how fast it progresses. how many years I have before I have bald head.



4. What is the cure if possible.



5. Does smoking aggravate hair loss. I smoke only avg 1 cig a day. And I tried stopping smoking for 3-4 months but didn't notice

6. Also advise how effective are Nizrol shampoo or Ketonext Coal Tar and Ketoconazole Shampoo
any significant difference so started again.

Hello. Thank you for writing to us

I have gone through your query in detail and I have understood your concern. I have also reviewed the Image.
I can see reduced hair density in frontal, mid scalp region in this image.

You definitely seem to have androgenetic alopecia.
Positive family history of androgenetic alopecia in your father is a marker of inheritance of this tendency to lose hair with age.
This type of alopecia is characterized by progressive hair loss and thinning mainly from the center of scalp i.e from, mid scalp and vertex of scalp.
Receding anterior hair line and widening and deepening of angles of anterior hair line are also seen in this type of hair loss.
Rate of progression of hair loss in androgenetic alopecia varies between individuals and cannot be predicted beforehand. Goal of treatment is to arrest its progression.

Topical Minoxidil 5% solution/ foam and Oral Finasteride are the only approved remedies for this type of hair loss in males.
Since you are already using Minoxidil therefore I suggest you to continue it. I also suggest you to add Oral Finasteride.
Together they produce much better results than either of these alone.

Goal of medical treatment is to arrest Or slow down progression of hair loss and to reverse early hair loss.
Therefore earlier the therapy is initiated better the long term prognosis.

Response to treatment varies between individuals and also depends on how early the treatment is started.

Your second concern i.e yellow fluid, crusting and itching from scalp and behind ear lobe, is part of seborrheic dermatitis.
Seborrheic dermatitis can contribute to hair shedding & decreased hair density in androgenetic alopecia, but solely on its own it is not responsible for a significant decrease in hair density, such as that seen in your case.

The shampoo that you plan to use i.e Ketonext shampoo, contains Coal tar and Ketoconazole and is definitely much better than Nizoral shampoo which is a purely ketoconazole based shampoo.
Though either of these can be used for seborrheic dermatitis and both are effective.
You may use this shampoo, every alternate days Or even daily depending on the response to treatment.

Smoking has no direct role in hair loss.
