Question: ok originally I had an intertrochantronic hip fractures in the femoral area of the neck. it was non displaced stable complete a very small fracture with an oblique pattern. it was still in position and alignment and nothing had moved. no bony fragments. it was not a communicated fracture. There was no injury that would have caused this type of fracture. I did not fall just a tiny slip on some wet grass. I had a d e x scan after the fracture with no signs of osteoporosis. at the time of the hip fracture I was not vitamin d deficient .

I had high blood pressure from taking a s s r I depression medicine that developed in the middle of 2012. I was diagnosed as pre diabetic this yearv in march of 2015. I was also diagnosed with low vitamin d. Never hadb that problem before because I was always out in the sun. since the hip fracture that occurred October 29th of 2013 I've been pretty much confined to the house with very little sun exposure. I have no diseases or health issues that would affect non union or delayed union. I only have a copy of cd disks with all the x rays c arm images mri images and c t images. I have actual reports read by the radiologist and medical records. The only medicine I was taking at the time of the hip fracture was effector ambien zyprexa xanax and tramadol. I have no family history of fractures or bone disorders and my bone density is good both positive in t and z. I have 8 siblings and 18 nieces and nephews and no one including my mother had or have any bone disorders osteoporosis or a history of fractures. I just don't know why my fracture would not union or delayed union and as of 6/16/2015 is still not healed. in October of 2014 I was just in the beginning stages of phase 2 of the healing process. I don't understand why the October 2014 c t scan showed butterfly communicated fragments and other fragments in the area of soft tissue with severe heterotopic ossification and sclerotic bone formation?

the doctor said I had sclerotic bone formation that developed in 5 weeks. I was under the impression that sclerotic bone formation eventually smoothes itself out but never goes away and never comes back.

the 12/4/2013 MRi did not show any sclerotic bone formation?

let me know if you have any questions. can you explain to me what can cause sclerotic bone formation?

Hello

I have gone through your findings in detail.
Sclerosis is localized or diffuse increase in bone density.It may be due to many causes.It may be due to both benign or malignant causes.
In your case,localized sclerosis may be due to slow healing fracture.
MRI is not very sensitive to detect early sclerosis.CT is considered as best investigation.
In case of bony injury,there is simultaneous bone resorption and deposition.If deposition is more than absorption then sclerosis may develop.
It is a very common finding in bone fracture.

Usually how long does it take to show up on an mri x ray ctrl scan or c arm if the fracture is five weeks old?

Is it common to see when a fracture is almost 11 months old? What type of image devices could show this and how does it appear on the images?

Dr XXXXXXX XXXXXXX what do you mean by in normal conditions?

Ok my mri was on 12/4/2013 and it did not show any sclerotic bone formation? It did not show any type of healing at all. 12/6/2013 I had open treatment surgery with rods and a dynamic hip screw in my femoral neck. The c arm before surgery did not show any signs of healing nor call us formation or sclerotic bone formation?

It didn't show up until my c t on 10/20/2014 and it showed butterfly communicated fragments and a fragment in the soft tissue that was stable. I was only in stage two of the healingprocess with severe heterotopic ossification and sclerotic bone formation. This was twelve months after the fracture occurred. I was only in stage two of the healing process with cells releasing to form call us.I was under the impression that sclerotic bone formation occurs once and never goes away and never comes back. I also had a c t on 6/13/2015 that still showed the fracture was not completely healed. The intertrochantronic fracture plane still had not fused. So give me your thoughts

My final question the fracture was five weeks old when I had the mri and surgery. I did not have any prior bony lesions no benign or malignant causes. No cancer at all. No diseases at all. No deficiencies at all no bone disorders etc. You said that an mri and c arm could both detect sclerotic bone formation or callus formation as early as 4 to 6 weeks in Normal conditions. I did not have abnormal bone deposition. I had a previous fracture and no problems with it healing. Do you think it was really there ? Do you think it was already starting to heal and the mri and c arm just didn't pick up any healing sclerotic bone formation or callus formation? It was only two days apart from the mri to the surgery and the doctor said it eas very difficult to access the proximal femur inserting the awl using the guide wire using the reamer and installing the rod in the femoral shaft. Just to mention this is not where my fracture was at. It was in the upper trochantronic region by the femoral neck. I really need some help. Do you think that the mri was wrong about the type of fracture I had and it was a communicated fracture with butterfly communicated fragments and a fragment in the soft tissue?

Normal conditions refer to normal patient.Callus is normally well seen in 4-6 weeks.
Your findings may suggest delayed healing.
Delayed/Non healing may be due to many causes like improper immobilization,infection,systemic illness etc.
MRI is not sensitive to detect sclerosis.
Your problem may be related to operative complications.It happens some times.
You may need detail evaluation and underlying cause of delayed healing should be evaluated.

I just emailed all the medical records consultation report and surgical report. Included in those emails are all of my mri results from 12/4/2013 my d e x a scan from 7/2014 my c t results from 10/20/2014 that I had to request from the hospital because Dr arebi did not provide me a copy when I requested my medical records. If you look at his medical record from July of 2014 you can see he did not mention any follow-ups. They did not call me about the res nor did they document anything in my medical records about the results. If you look at the 6/3/2015 medical record you can see they even left it out of there. They used the comparison from the 4/8/2014 ctrl scan
This is the report I am must concerned about because it indicates butterfly communicated fragment and fragment in soft tissue. An rd butterfly communicated fragmenting. This looks like it is a different type of fracture than what I previously had. If you also look at the c t results form 6/13/2015 you can see I I was just in stage 2 of healing with callus formation with severe heterotopic ossification and sclerotic bone formation? I thought you only get it once and it never goes away and never comes back? I will upload the images soon.

Hello

I have gone through all reports in detail.
You are responding well but healing process is slow.As we discussed earlier,delayed healing may be due to many reasons and some are inevitable reasons.
It is really difficult to manage comminuted fracture and align all fragments.Comminuted fracture,obesity,metabolic causes etc may be the reasons for delayed healing.Butterfly is a type of comminuted fracture.
You should allow some time and keep trust in your doctors.Latest CT report show improvement.

I did not have a communicated fracture see mri report from 12/4/2013. I had an intertrochantronic right hip fracture that was non displaced with edema. it was stable oblique pattern very small similar to a stress fracture. it was a complete break but still in alignment nothing had moved out of place and in good position. that's my question? the mri was done on 12/4/2013 in a open high field Hitachi 1.0 machine. it did not show any type of communities fracture or fragments. it did not show any signs of healing and it did not show any sclerotic bone formation. I do not have any diseases disorders osteoporosis multiple sclerosis problems or pageants disease no cancer nothing at all. no vitamin deficiency bone density is healthy above both normal ranges in both the t score and z score. the only mental health issues I had were mood disorders and high blood pressure. I weighed 236 pounds@the time of the surgery but the hospital weighed me wrong. I saw my primary care doctor on 11/26/2013 @ 11/8/2013 and I weighed 236 pounds. im trying to upload the actual mri x rays c t scans to you but I haven't heard back from customer support on how to zip these images from my scan disks and upload them. as you may know and mri takes many images and so . do c t scans. maybe you can help with contacting someone to call me. 513 330 3402.

CT findings in 2014 shows comminuted fracture where as MRI in 2013 didn't comment about comminuted fracture.As definite cause of fracture is not known in your case,development of spontaneous fracture chip in 2014 may be related unknown etiology.
