Suggest Treatment For High BP

Question: "i was on Revlol AM 25/2.5 twice daily plus cardace 5 mg for the last 7 years. Before that I was on lower dosage.in the last 3 months, my BP has gone up to 160-170/ 90. My Dr prescrbed Telma AM ( 40 mg & 5 mg) and Metroprol 25 mg, both twice tablet. still the BP has not really come down, though i have been taaking it only for the last 3 days.

Pl let me know if the disage is very high, compared to what i used to take. Pl reply

Since your BP was not controlled with Revlol AM 25/2.5 twice daily plus Cardace 5 mg once daily hence your treating physician has now changed the medicines.
Instead of Revlol AM, Telma AM 40/5 & Metoprolol 25 mg twice daily.
This new combination is effective and safe in controlling the high BP and the dose prescribed is adequate.

Thanks Dr. for your reply. however pl reply to my secific question- is the dosage Telma AM 40/5 & Metoprolol 25 mg both twice daily, a high dosage in general for BP . will there be any side effects.
Also the issue is before changing, also the reading was around the same - say 160/94 . I first tried various combinations as directed by Physician over the last 1 month. First Dr. made it to plain Revlol 50 mg & cardace 5 mg in the morning and in the evening Revlol 25 mg, caardace 5 mg and LN Block 20 mg one tab each. again the BP variation was there around 150-170 systolic and dystolic around 84-96. since it is still high some 2 days back, he prescribed Telma AM 40/5 & Metoprolol 25 mg both twice daily.

I feel quite uncomfortable with these changes and still BP has not settled down. i am also bit anxious and this also is a cause.

I can understand your situation.
Usually Telma-AM (Telmisartan+Amlodipine) is prescribed ONCE daily for the treatment of high BP & the maximum dosage for this combination is Telmisartan 80 mg/day and Amlodipine 10 mg/day.
Similarly, the maximum dose for Metoprolol (which is also prescribed ONCE or TWICE daily is up to 100 mg/day).
So, the prescribed dosages for these medications is within the recommended range for the UNCONTROLLED BP.
If you experience the side effects like dizziness, constipation, swellings around the ankle, fatigue, cold and clammy skin these should be brought to the notice of your treating physician.

Thks Dr. for your answer. i have been on the new medicine for 4 days- since 19 th Nov. the BP is now around 150 / 85 and i think it is coming down. When the BP reading is taken , i also have some bit of anxiety, so I presume my normal reading is around 145/85......

Can I cut my dosage to Telma - AM morning 40/5, and evening 20/2.5 and take it along with Metroprol 25 mg twice a day. Will it be fine. Eventually if the reading is normal within the next 1 month, I want to keep it Telma AM 20/2.5 & Metroprol 25 mg twice a day. I will be comfortable with this mentally.

Also my age is 61 years, had an angioplastry in 2007 for 1 stent done in anticipation. OtherwIse I have no complaints, and normal life incl walking, no bad habits. For my age can I take the normal reading as 150-160/ 90. No sugar.

I have gone through your reply & I can understand your concern.
Yes, in order to get the accurate measurement of the BP, one should relax for at least 30 minutes.
Since you have already undergone angioplasty in 2007, hence the BP of 150-160/90 mm of Hg can not be taken as normal.

Regarding the dosage reduction, since you have just started taking it a few days ago it is not advisable to cut the dosage. Kindly take the medications in the prescribed dosage for at least 2-3 weeks, then take an opinion from your treating physician for the possible dosage reduction.
