Question : Hi My 18 months old son has 41degree fever. He is taking medicines. But he shudders and grind teeth . He is a very poor eater. Since the time he has been having fever he does not eat anything. Occasionally he asks for water and take a sip or two. Please advise reasons Thanks

Brief Answer: Get CBC and urine examination Detailed Answer: Hi Welcome to Healthcare-Magic Greetings of the day Dear Mr XXXXXXX As your son is having high fever, one needs to find out the cause of fever. In majority of cases the cause is most likely to be Non specific Viral infection. I would like to know whether he has other associated complaints 1. Any cough , running nose , sneezing or any associated respiratory symptoms 2. Is he urinating well:any excessive crying while passing, is the urine foul smelling? 3. Any gastrointestinal symptoms like loose motion, vomiting 4. Is he sleeping well ? 5. Any skin rash? Grinding of the teeth is known as trituration. It usually occurs in high grade fever, in the absence of abnormal movements in rest of body there seems to be no cause for concern. In the meantime I would suggest you to continue giving Paracetamol and also ensure his a adequate intake of food and water. Giving warm tomato or chicken soup , coconut water, wheat porridge will be helpful. I would also suggest to get the following investigation done 1. Complete Blood count 2. CRP 3. Urine examination : Urine routine and microscopy examination Rest be assured things can be taken care off. Do get back to me with details. Thank you Dr XXXXXXX for the advice. As regards your questions I give the answers below. I shall also get the tests done and intimate you the results. 1. The boy generally sleeps for one to one half hours in the day time and in the night wakes up once or twice. After he is sick he does not sleep during g day and has very disturbed sleep during night. It has become very difficult to feed him and he does not like anything other than orange. 2. His urine is odour less and he he does not cry while urinating. 3. He has mild cough and heavy running nose. He vomited once or twice in the last two days during night but not today. The vomit was whatever little food and orange he consumed. 4. He does not have any loose motion or respiratory difficulties 5. He does not have any skin rash He is on following medicines 1. Tixylix for Clough 2. Meftal for fever 3. Augment in DDS (Amoxycillin and potassium clavulanate oral suspension IP) 4.Lanzol junior one daily. Should we continue this or stop. His fever has come down Thanks once again 5

Brief Answer: Steam inhalation will be helpful Detailed Answer: Namasthe Dear Mr XXXXXXX As he has running nose and cough, the cause of his fever is likely to be upper respiratory tract infection. Babies are usually irritable during upper respiratory infection as they tolerate nasal block to lesser extent compared to adults. Sleep is also sometimes disturbed due to cough and nasal blockade. There is no cause for concern as majority of times the respiratory infection is caused due to virus and is self limiting. If the child is breathing comfortable there is no cause for concern. I would also advise you the following which I use on all my Surgical and Non Surgical Pediatric kids and angels. 1. Steam Inhalation: 10 minute twice a day . Using electronic steamer will be beneficial. 2. NEBULIZATION: it is always advisable to have a Nebulizer machine at home, as will be helpful as children do develop repeated respiratory infection. Normal saline nebulization will be helpful. 3. Antibiotic are generally not needed as it a virus infection 4. Decoction : A cup of milk + 10 Tulsi leaves + 2 gms of fresh ginger + 2 black pepper + 1 spoon of turmeric + 2 spoon of sugar. Bring it to boil. Filter it and feed the baby warm with watti and spoon. It will help in relieving congestion of throat and also nasal blockade. 5. Continue the cough medicine 6. Warm food and fluid; warm tomato or chicken soup with little black pepper added will be helpful 7. To a spoon of honey add 2 drops of fresh lemon juice and give him thrice a day.
