Suggest Treatment For Incomplete Bowel Movement And Constipation

Question: I have been having problems with constipation for about 4 years, getting worse, not hard, just incomplete, started having pain in RUQ, ultrasound for gallstones was negative, had baseline colonoscopy 4 years ago, negative, had again XXXXXXX 1, negative, GI dr sent me to gyno, thought it might be rectocele, or cystocele, gyno says no. I am taking linzess once a day, and miralax 4 times a day, for a while, though i am going i am still having the RUQ pain, it is intermittent. my back has been hurting as well. i drink plenty of water, eat plenty of fiber, exercise alot, I am 162 lbs, 5 ft 4 inches, I am starting to think we are looking in the wrong places, something else, mybe not GI causing both the RUQ pain and sluggish bowels

Incomplete bowel movements create a persistent discomfort, affects concentration in day to day activity. I understand your concern.
- I have read your query carefully, You have been investigated for probable cause of pain in RUQ by USG, got colonoscopy done, got examined by gynecologist to exclude rectocoele.
- You are taking Linzess ( a stool softner & laxative ) & Miralax ( a laxative stimulating bowel movements.) daily. Naturally you don't have hard stools, you have soft stools , but feeling of incomplete bowel movement.
- It's good you are conscious about taking fibers/ lot of water & exercise .. all of these are is good in managing constipation.
** Still I feel few things ( which can affect intestinal activity)remained to be checked,
* Infection / infestation of intestines-Constipation/ infections/ infestations tend to depleate normal bacterial flora of intestines ( Lacto. Bacillus) - which help intestinal infection to flourish & viseversa
Get your stools routine & microscopic tested for presence of bacterias, amoebas, worm infestations.
- You had taken some drug ( Amitiza ) for amoebiasis. But it tends to recur/ get chronic.. causing increased mucus secretions in bowels ,..make stools more sticky leading to constipation & incomplete bowel movement.
Actually,- repeated courses of medications/ avoiding recurrence of infection by good personal, serving hygiene / drinking boiled water .. need to be followed to get rid of such infections/ infestations.
- Intestinal infection/ infestation causes digestive deficiency. leading to collection of gasses under diaphragm.. this causes pain in RUQ.
* Forming regular habit of bowel evacuation.- Irregular eating & bowel evacuation habits can cause constipation./ incomplete bowel evacuation.
- Psychological cause- Voluntary withholding of stools ( due to fear of public toilets, laziness, too much occupied routine ) can lead to constipation.
- It is better to manage with high fiber & water intake/ abdominal exercises than regular use to laxatives.
* Conditions like diabetes/ hypothyroidism - lead to sluggish intestinal movements & constipation/ incomplete bowel movements.
Thyroid function test/ glucose tolerance test would exclude/ diagnose these conditions.. & needful treatment would solve your problem.


In this context I would advise you to consult your family physician. He/she will get you investigated & specifically treated to remove basic cause of the constipation. He may refer you to Gastroenterologist/ endocrinologist as per need,for special consultation.. This will help you in long run.
