Brief Answer:
fungal infection most likely. please send photographs. apply antifungals.

Detailed Answer:
welcome
i understood your query.

i request you to send recent photos of scrotum and penis.
i would like to know whether you are suffering from diabetes or HIGH BP?

Itching can be due to dermatitis , or fungal infection or infection like herpes.
have you redness around scrotum? is there any type of discharge from penis?

It is more likely of fungal infection.

Tinea cruris is a type of fungal infection which is supposed to be very irritating and difficult to treat because the groin area gives it perfect environment to grow.
Even if treated and patient feels it is gone, it has not. Even few threads remaining may grow again like the original one. fungal infection of foot is called tenia pedis.

I advise you to avoid tight clothing any time of the day and no under garments as and when possible, cotton is suppose to be better to absorb sweat. Keeping the affected area ventilated is the idea. you can apply calamine lotion which acts as a soothing agent.

A cream containing broad spectrum anti fungal activity and steroid (to reduce the inflammation) need to be applied thoroughly.

Hope this will be useful.

i am here to give you answer.
thank you

Dear doctor,
I uploaded photos of scortum penis and buttock also feet. Please note that my BP is normal and I do not have diabetes. Also there is no redness in the affected part. Some times the itch sensation is so much that the affected area becomes red.. After your advise I am applying candid b cream (clotrimazole + beclomethasone) . please let me know if I need to apply this both on scortum feet and buttocks.
Regards,
XXXX
