Question: Have itchy red bump with black dot at center on forearm. black dot removed by squeezing bt itch remais.

Its single rash which describes its cause is a local one ( may be some specific exposure, insect or bug bite which is more likely with a rash which you defined, due to scratching, hair follicle infection or blocked sebaceous gland).
Its difficult to diagnose skin conditions without actually seeing, but I have a vast experience in successfully treating such cases.
Dont scratch on the area any more. Avoid any allergen your skin may be sensitive to.
I usually recommend applying hydrocortisone cream on the lesion 3 times a day, and Tablet Benadryl 50mg twice daily per oral for 5days.

It is not a rash. It is a raised reddish circular area about 15mm diameter with a distinct center which oozes slightly where I squeezed out the black 'core'. Had same 'bite' on other forearm a week ago. Believe it is a bite or sting but by what?

It's a bite, and definitely starts with a rash and forms a bump.

I welcome you back and appreciate the cooperation by telling that it's a second episode. So, I suggest you to change the bed mattress and ask for some pesticide spray every week at least 3 times in your home. The Hot and humid environment in rooms causes bed bugs ticks and mites to develop and bite.

Use a humidifier and keep humidity below 55 in room.

Treatment is same as aforementioned. If you see pus coming out of the mouth of this circular lesion, better call it a pustule, showing its infected now, then please use Tablet Augmentin 625mg twice daily per oral for 3days.

Definitely NOT bedbug bites! The two bites were individual incidents and do not resemble bedbug bite photos. The BLACK CORE is the significant feature and does appear in bedbug bites.

Skin lesions are difficult to treat without actually seeing them. In the last answer I gave two options to you and suggested separate remedies for both.

Possibility of 1. bedbugs and 2. a pustule that may be formed due to infection of the hair follicles ( folliculitis), sebacious gland blockage ( sebacious cyst) etc etc.

Every patient uses different words in history. We the physicians, listen carefully and regard that. But after physical examination we can reach to a diagnosis.

If you were still in doubt, your concerns are genuine and I will appreciate a little more cooperation if you please attach some high quality pictures of the lesion. It gives very important clinical clues to a physician especially when practicing on a virtual clinic.

Its a type of urticaria. Needs treatment, and precautions.

I really appreciate your compliance and support in terms of providing picture of the lesion in a timely manner.

So, its clear from the picture attached, that this is a recurrent 'papule' caused by a hypersensitivity reaction (body responds to the foreign invaders) to the bites of mosquitoes, fleas, bedbugs (you ruled this possibility out in our discussion above) and other Arthropod insects.

The individual papules mostly surround a reddish wheal and display a central punctum (The black dot in the centre and it oozes when you squeeze it). We call it a punctum.

It is a type of skin allergy (Urticaria). Its common in spring and summer seasons.

The patients with this disease when present with these symptoms, I usually prescribe them:-

1) Elocon Lotion to be applied topically twice daily.
2) Tablet Allegra 1 daily by mouth.

I follow up usually in 2 weeks and if there are any signs of secondary infection, antibiotics are added to the prescription.

Cautions:-
Important is to use insect repellent lotion when you go outside.
Please also use flea and tick control on your pets (if you have any indoor pets).
DEET containing insecticides are beneficial. Insects can't bite you then.
