Suggest Treatment For Itchy Skin And Redness On Some Parts Of Body

Question: I am suffering from skin allergy,many type of medicines I tried currently I am taking Montimac FX(Molukaeast Sodium & Fexofenadine) and Defcort 12mg(Deflazacort) but not working this time.I am suffering from itchiness of skin and redness on some parts of body.I am also a peptic/Duodenal ulcer patient from two years I am taking Rebeprazol & domperidone tablet everyday morning.Please suggest for any type of test and please help me.

The condition you have is likely chronic (idiopathic) urticaria, and the gastritis due to the infection with Helicobacter pylori worsens urticaria in some people.
In that sense, the word idiopathic is put in brackets.
Also, it appears that you have tried several medications in the past, but do not know in what combinations. Whatever the case, you have to slowly taper and stop taking steroids as you have now become dependent on steroids.
Chronic steroid usage does not help with the gastritis.

You will need the following checks and tests:
1. Blood pressure
2. Complete blood picture, LFTs, Antinuclear antibody, Uric acid, fasting glucose and lipid profile, Vit D levels, Urea Creatinine, Na and K, calcium levels
TSH, fT4 including anti-TPO and TG; mast cell tryptase (if locally available)

Treatment:
Taper Defcort by 4mg every week and stop
Start Zinetac 300mg before breakfast
Allegra 180mg after breakfast
Cetzine 10mg at 2PM
Atatrax 50mg at 10PM
Use Dermocalm lotion as required

After the blood results are ready, I will consider starting Ciclosporin if there is no change in the condition.
You will need to also upload a picture of the rash when it is at its worst.

Dear sir I started your suggested medicines but not working too much.Itching problem happening after using medicine.Skin redness is simple red like common allergy conditions.My right four finger have a fungle infection from three to four years,He increases and decreases himself.My memory is very week from past three four years If anybody asks do you eat then I have to think some time for answer the common simple question.I forgot things within 5 minuts.I am Sending some past reports please see. I contact some labs like thyrocare and SRL Diagnosis but MAST CELL TRYPTASE test not available here. can my problem identified without this test or will I come to meet you I am from XXXXXXX XXXXXXX Can you meet another place except Apollo Hospital If yes which timing.

Mast Cell Tryptase is available at Apollo Hospital XXXXXXX and is important to know if this is normal or abnormal.

Dear Sir I am sending all your suggested reports, see this and tell me about my problem and start medication.
From last 10 days my both hips are too much itching I am using Dermocalm lothion but not so much working.skin is slightly looking fat.After itching many places skin erased. Another problem is My memory is very week from past three four years what the reason please say some also about it.

Many thanks for uploading the reports.

Apart from raised mean platelet volume - that is now an indicator of uncontrolled urticaria, and significant Vit D deficiency, slightly high total cholesterol values; the rest of the results are normal.
I did not see C3 C4 values.

Given your history of severe acid peptic disease, relative lymphocytosis on blood counts, it would be worthwhile to get an upper GI endoscopy done after consulting a Gastroenterologist.
H pylori infection can cause difficult urticaria, and I do not want to consider this treatment option unless we are sure you do not have this bacteria.
Continue on the antihistamines, you can take extra cetirizine as and when required.

Why you are not starting my treatment after all test done.you said earlier after blood test results ready you start cyclosporin if there is no change in the condition.Sir I am not feeling well.I am Irritating by itching.I want permanent treatment of this problem. I thought that I will cure after blood tests by your treatment.You can't advises any medicine for Vit D deficiency,Negative ANA etc.
Before 18 months my endoscopy done and detected H-pylori infection.I am sending report and prescription.Can I do endoscopy again.

Treatments such as ciclosporin are immunosuppressive agents and you need to be informed of all risks and benefits before starting this therapy.

Secondly, there is a significant proportion of patients who fail 1st treatment for H Pylori and may need to get a 2nd clearance therapy, hence the reason to consider a 2nd UGI Endoscopy.
