Suggest Treatment For Lower Back Pain

Question: Hello Dr. Rynne! I've been having a lower back pain for the past 2 weeks and it won't go away. It is not a continuous pain, but a deep one. It hurts when I lie down in in bed and tried to change sleeping positions in particular, very small movements, Sometimes it could hurt when I walk, but rarely. It does not hurt when I am standing. I exercise regularly, although I tried not to jump or do too many hard core cardio activities lately, but I have't stopped exercising. I am an almost 43 years old woman in good shape and I am not overweight. Your opinion is much appreciated. Many thanks.

I have read your query and understood your problem.

It appears that your are suffering from low back pain for quite since time. The pain is present only on the back and does not radiate to the lower limbs. This goes in favour of a few possible differential diagnoses like Mechanical back pain which is back pain due to bad posture while working or sleeping, Vitamin D and Calcium deficiency such can cause pain in the back and muscles, wadi bones due to Osteoporosis due to Menopause and related hormonal disorders, Pinched neve possibly due to slipped disc such can also cause shooting pain from the low back to the lower limbs with pins and needless sensation.

I would advise to adopt good posture while working. Kindly follow ergonomics. Sleep on a stiff mattress that does not yield to much pressure like an Orthopaedic mattress. Apply hot water bottle of warm soaks to the region of pain to reduce the pain due to muscle spasm.

Take Diclofenac 75mg or 100mg and Thicolchicoside combination tablet twice a day for pain relief of alternatively the same drug combination in the form of topical gel for local application to the back. Continue exercising cardio for about 45 minutes a day by brisk walking. Eat healthy.

Avoid smoking and drinking alcohol. Get adequate sunlight exposure and consume milk, cheese, nuts, cereals, green leafy vegetables for adequate calcium and Vitamin D levels.

You can wear a lumbar belt such will offer relief while sitting or standing. Avoid bending forwards or lifting weights.

Continue this for a week. If no improvement then you need to consult your Orthopaedic surgeon to rule out most of the above mentioned diagnoses with proper clinical examination and necessary investigations like Xray or MRI as necessary.
