Brief Answer:
Pls do lipid profile test and consult cardiologist

Detailed Answer:
Hi XXXXXX,
Thanks for writing in to us.

I have read through your query in detail.
Being a radiologist, I see fatty liver grade one in many patients each day. It can be due to increased fat consumption, alcohol intake and also a normal occurrence in some people who are over weight or have a family history. Pancreatic changes is also due to fat and alcohol intake. These changes are not overly concerning but I will suggest the following:

1. It is important to know your blood cholesterol levels and I suggest you get Lipid profile test done and discuss the report with your doctor. The lipid profile test result will tell the cholesterol levels in your blood and the risk of heart disease.

2. As you have not mentioned high blood pressure and diabetes, I guess you do not have those conditions.

3. Since your liver is also enlarged in size (hepatomegaly) it will be nice to also do complete liver function tests and see for any raised enzyme levels.

4. At your age, consultation with a cardiologist after getting lipid profile test report and getting an ECG or echocardiography will help to know any risk of heart disease and if you need any medications should you have high cholesterol levels.

5. You must start on a healthy low fat and diet and also include meditation and walking for 30 minutes every day alternately.

The simple liver cyst if small is not a concern.

Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek

Brief Answer:
Heart disease risk high at triglyceride 320 mg/ dl

Detailed Answer:
Hi XXXXXX,
Thanks for writing back with an update.

The heart disease risk is high when I see your triglycerides and VLDL levels. It is best if you consult a cardiologist immediately and get a complete heart check up done. An echocardiography will tell more about the heart functioning and then if necessary further investigation might be required. RepaceH is a good medication for blood pressure control and please also discuss with your doctor.

Sugar levels are acceptable.

Diet changes should be made as mentioned earlier.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek
