Question: Hi Doctor,
My RBC count and Hematocrit is higher than the average value. Please find the attached reports from past two years.I am feeling muscle fatigue and tingling feelings in my leg and hands very frequently.I checked with many neurologists for this problem and they told I dont have any nerve problem after all the tests like MRI,NCS all came normal. My question is,

1.Is the high value of RBC and hematocrits causing this muscle fatigie and tingling feelings ?Its reallly making my life miserable now as I cant even stand for 10 min because I feel pain in legs after that.Night I get up many times because of the tingling feelings and blood circulation disturbances.
2.Is this a symptoms of something serious that may happen in future ?
3.What should i do to reduce the RBC count and whether its needed ?
4.I am on medication for anxiety and depression like prothiaden, zylone forte.Should i take any other medicine for this problem ?

Brief Answer:
Its not polycythemia.

Detailed Answer:
Hi,
Thanks for asking.
I am Dr. Jyothi BL, and I will be answering your query.

For your queries, my opinion is as below:

1. The high value of RBC/hematocrit is not responsible for your symptoms. If the value was above 20 g%, it could be the cause of thrombosis/infarction. Not muscle fatigue or tingling. This is seen more in neurological causes, maybe nerve compression, degeneration or nutritional deficiencies. If the pain, you are feeling is throbbing type, then it could be due to peripheral vascular disease.

Are you a smoker/ alcoholic. Do you have any lung problems or exposed to too much air pollution or do you suffer from any chronic diseases? I will require answers for these questions.
Is there any swelling in lower limbs? Have you got any superficial veins in legs? Any color doppler study of lower limb done?

2. Symptoms are minor. As you are well evaluated, I dont expect something serious to happen in near future.

3. Blood reduction not necessary. However, donating blood is a good idea.

4. I would advise you to take a nutritional diet, walking/jogging/aerobic exercises to improve circulation. Nutritional supplement composed of B-complex vitamins with zinc, selenium, magnesium, antioxidants like lipoic acid, etc will be helpful. Trace vitamin deficiency is hard to diagnose. Also add in your diet - fish, eggs, Vit C rich fruits like oranges, etc.

Earlier reports are normal. Not to worry.

Hope it helps.
Any further queries, happy to help again.

Thank you Doctor.

I used to smoke but left two years back.I drink Occasionally now.I dont have any swell in my leg, hands nor have any chronic disease. From past 10 years I have these problems and as I am not able to diagnose anything,I am going into depression and past three years,I am on anxiety and depression medications. But worst past is these symptoms never goes or reduce but increasing with time.
If its not neurological as doctor are suggesting I am not sure what is going on .

Do you think of any other possibility, that I may consider ?

Thanks,

Brief Answer:
possibly nutritional

Detailed Answer:
Hi XXXXXXX
Thanks for asking again.

Two years back the hemoglobin was also more. Further air pollution has further contributed to further lung damage, possibly leading to further increase in hemoglobin.

Tingling is always neurological. If its progressing, it could be further degeneration. Possible causes include nutritional most commonly. Improve your nutrition with b-complex vitamins and zinc plus selenium. There are multiple minor nutrients which are necessary for neurological growth. Concentrate on supplements and nutritional diet.

Also add few antioxidants in diet like green tea, lipoic acid, vit c, etc. It will slow neurological damage.

However, vascular causes, even though rare needs to be kept in mind. Improve circulation through aerobics, jogging, and other exercises.

Drink plenty of water.

Overall, if followed for a week should show few signs of improvement. Progression will definitely slow down. Reversal is possible. Keep your weight within normal limits.

At present thinking of nutritional deficiencies leading to progressive nerve damage.

Hope it helps.
