Suggest Treatment For Nausea And Stomach Pain When Treating GERD

Question: My 93 yr. old mother had the flu a month ago. She can't seem to get over the nausea and stomach pain. all other symptoms have subsided. She has heart issues, which have her on Coumadin, and she has been on Prilosec for over a year to deal with GERD. She is losing eight that she can't afford to lose, and can't keep anything down for long, thus getting minimal nutrition. We get her to eat things like yogurt (She doesn't have dairy issues). Is there anything we can try, or should we be getting to the Dr. for additional eval?

Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.

Based on your query, my opinion is as follows:

1. Nausea and stomach pain appear to be related to reflux esophagitis or GERD.

2. Additionally, to Prilosec, which is a proton pump inhibitor, she will need to use domperidone - which will reduce acid reflux. Prilosec contains omeprazole, it can be used in combination with domperidone. Alternately, Pantoprazole with domperidone like Pantodac DSR can be used.

3. Use of domperidone, will reduce nausea and hence any vomiting.

4. Avoid spicy or sour food. Give her good amount of milk products, cold milk, milk shake, ice cream etc. After food, erect position is essential for two hours. Mild walking will be helpful.

I am not sure, she cannot keep anything down for long - you mean to say she is vomiting. The combination medication above will help and needs to be taken twice daily half an hour before food. She requires constant support at present. Good hydration is also necessary.

Look into stool color - is it dark black or red. If not, good - No bleeding.

Even after a week of this medication, if issues persists, an gastro-esophageal endoscopy maybe necessary.
