Brief Answer:
oral thrush (antibiotic induced)

Detailed Answer:

Hi,

I had gone through your question and understand your concerns that you are having throat irritation with coated tongue. I would like to ask you few more details.

-Is there any nasal obstruction or running nose or continous bouts of sneezes ?
-Is there any post nasal drip (nasal secretions dripping back into throat) ?

Kindly get back to me with the details.

Based on your clinical details and photos, it appears to be oral thrush (fungal infection of tongue with sinusitis (x-ray).

I suggest you to follow these measures to overcome your health issues.

1. You need tongue scrapings for culture and sensitivity (fungal infection) and CT scan of para nasal sinuses (4 month sinusitis).

2. Kindly discuss with your treating doctor for the aboue investigations.

3. In the mean time, do salt water gargle twice daily, make a habit of cleaning the tongue surface.

Hope this answers your question, if you have additional questions or follow up questions then please do not hesitate in writing to us. I will be happy to answer your questions.

Wishing you good health.

Brief Answer:
needs ct scan of para nasal sinuses.

Detailed Answer:

Hello again,

Thanks for considering my answer and for your follow up.

Your symptoms denotes allergic rhino-sinusitis with ear drum infection with or with out perforation in ear drum.

Allergic rhinitis is a chronic condition, can be confirmed with detailed history and blood tests (absolute eosinophilic count, serum IgE levels) and ct scan of para nasal sinuses. You may require life time treatment to prevent it's complications (sinusitis, polyp or ear drum infection). Kindly go ahead for the investigations, what I have already mentioned.

To control allergic rhinitis, you must follow some preventive measures, some are- not to expose to environmental cold, dust, not to consume freezer products (curd/ice cream), wear helmet during bike ride, avoid inhaling strong pungent smells (deodorants, kitchen odors) and avoid water per ear (plug them with dry cotton during bath).

Hope, I have answered your query.

Take care.

Brief Answer:
complete blood picture-allergy.

Hello again,

Your CBC (complete blood picture) suggestive of allergy (raised eosinophil count). You need to undergo above tests to see the degree or magnitude of allergy and to confirm the complications.

Brief Answer:
maxillary sinusits- secondary to allergic rhintis.

Detailed Answer:

Hello again,

1. As per your CT report, you have got deviated nasal septum, turbinate hypertrophy with maxillary sinusitis. If your symptoms are more than 6 months, sinus surgery is the treatment of choice.

2. Chronic post nasal drip ( sinus secretions from nose to throat ) could be the cause for your ulcer over tongue.

3. Chronic ulcers needs to be evaluted with scapings for culture (fungal ) or biopsy. Kindly go ahead.

Hope, I have answered your query.

All the best. Take care.

Brief Answer:
film needed.

Detailed Answer:

Hello again,

Kindly scan the film of ct scan of para nasal sinuses (depth of the disease decides the treatment modalities).

Yes. Possible. Previous external trauma can cause deviated nasal septum, which can lead to sinusitis.

Take care.

Brief Answer:
prophylactic anti allergic medications.

Detailed Answer:

Hello again,

Based on your CT scan film, you are a case of sinusitis secondary to allergic rhintis. In my opinion, a course of antibiotic, prophylactic antiallergic medication like montelukast and steroid + antihisatamine nasal spray for 3-6 months will be benificial in your case.

As I, have already mentioned you, people with allergic rhintis, needs life time medications (nasal sprays and montelukast) and regular follow ups to prevent complications (sinusitis, polyp and recurrent ear infections).

Kindly discuss with your treating doctor for dosage, strength and duration, as they are prescribed medications, needs to be taken under supervision.

All the best.
