ok , so here is my situation :

I have lots of female sex partners , But I always use condoms ( for oral sex as well most of the time ) . About a 2 months ago I had protected sex with Cuban sex worker in Cuba . Unfortunately condom broke during vaginal sex . I can't confirm her hiv status. It was only for 30 seconds or so.

After I noticed the condom broke , I ran to bathroom , I peed and I rinsed my penis with mouth wash ( that's all I had available )

Anyway I did a RNA test about 8 days after exposure for Early detection
It came back negative .

However for past month or so aside from dealing with anxiety ,
I am feeling little discomfort under my left armpit .
I was at doctor and he ruled out Lymph node. ( But I never told him my HIV Fears ) This discomfort sometime travels down my arm little bit to my
elbow are . I cant really confirm what that is . I am thinking maybe its
joint pain related to HIV ??

Anyway , this morning I took my wife to ER she had hip surgery 3 days ago
and her heart is skipping and some other issues she is having .
But what freaked me out is that the doctor said that she has Thrush .

When I heard she has thrush , my heart dropped to my ankles.
I am freaking out thinking I got HIV and now I gave HIV to my wife .

I am freaking out that this could be the signs of HIV . Does this sound like a HIV possibility ? I am super scared of taking another HIV test fearing it may come back as POSITIVE .

Help .....

Answered by Dr. Sunil Jangid (6 hours later)
Brief Answer:
The discomfort under left armpit and thrush is not related to HIV.

Detailed Answer:
Dear XXXXXXX

You had multiple sexual partners and protected sex all the times. The condom was broken once and the exposure was just for 30 seconds then the possibility of getting HIV from such exposure is very less even if the girl was positive. HIV is relatively hard to transmit and the risk of HIV transmission from female to male during unprotected vaginal sex is only 0.04% so don't get scared.

As far as discomfort under your left armpit is concerned it is not related to HIV. It can be due to lymph node as diagnosed by your doctor or some other reason. Your HIV RNA test after 8 days is also negative so don't worry because the risk is very less. The thrush is mostly due to some fungal infection not related to HIV for which your wife can take treatment as suggested by the doctor.

Having multiple sexual partners is very high risk behavior for HIV I will advice you as your friend to stay away from such unsafe practices to prevent the risk of HIV or other STDs in future. Don't get feared and get tested after one month with 4th generation test to be 100% sure about your status. The possibility of result being positive is very rare so be relaxed and move ahead.

Let me ask you ,
Everyone says having many sex partners is high risk yet you tell me that my risk of exposure was minimal . I am a bit confused .
I had probably about 100 sex partners and this was my first time a condom broke , that potentially put me at very low risk.

So I'm confused why all doctors say it's high risk behavior . I can understand that because of my behavior I am writing to you this letter cause
Of course I do not want to get HIV , so my anxiety is running wild . Certainly I am paying
The price for the fact I got drunk and careless
And I promised my self not to drink and have sex
With hookers next time .

Anyway, can you tell me if you recall out of many many cases of HIV infections that you have seen ,
How many got infected with HIV by having a single episode of unprotected sex ?

doctor
Answered by Dr. Sunil Jangid (12 hours later)
Brief Answer:
The risk of getting HIV from single episode of unprotected sex is very less

Detailed Answer:
Dear XXXXXXX

Having multiple sexual partners is definitely very high risk behavior for HIV as I had told earlier also but at the same time it is also true that the possibility of getting HIV from single episode of unprotected sex is very minimal and I have not seen any such cases so don't get scared or worried as the risk is very less.

Thank you doc

I appreciate talking with you about my issue .
I have been living with this anxiety from this one single episode for exactly 2 months and two days

Perhaps doing the test 7-8 days might have been premature ,and I want to put my anxiety to rest once and for all . I decided to take the HIV RNA test once again . After 2 months this test should be rock solid that I am still HIV negative . Yes?

They say this test should be able to detect HIV as early as 7days . I will be going there Monday morning and results should be back by Friday .

Would you like me to keep this query open till Friday and let you know what results I got ?
doctor
Answered by Dr. Sunil Jangid (55 minutes later)
Brief Answer:
You can keep this query open till the result of HIV RNA will come on Friday

Detailed Answer:

Let me ask you something else
Everyone says your chances of getting HIV
Are higher if you have other stds.

I have herpes 1&2 , I never have breakouts ,
Time to time I feel like an itching sensation by my scrotum , but I don't ever recall getting any blisters .

Also , I have genital warts . Same thing I had few
Removed years ago and they didn't bother me since .

Does this effect my odds of getting HIV ? And by how much ?
doctor
Answered by Dr. Sunil Jangid (2 hours later)
Brief Answer:
STIs like herpes, syphilis can increase the risk of HIV transmission.

Detailed Answer:
Dear XXXXXXX

Sexually transmitted infections like herpes, syphilis can increase the risk of getting HIV. It is estimated having genital herpes makes a person much more susceptible to HIV infection. The best way to clear all doubts is to get tested so don't worry so much and go for HIV RNA test with hope for the best.

So if you have herpes 1&2
Not necessarily you are having an outbreak ,
You are more susceptible For HIV infection ?

So normal chances for transmission are 1 in 1000
Per exposure how do the odds change for person
Who is infected with herpes ?
doctor
Answered by Dr. Sunil Jangid (7 hours later)
Brief Answer:
If you have genital herpes and exposed to HIV than the chances are more

Detailed Answer:
Dear XXXXXXX

If you have genital herpes in present exposure than the chances of getting HIV are somewhat higher 2 to 4 per 1000. Don't get so much worried or tensed and get tested, it will clear all your doubts and the result will come in your favor so just relax.

doc .

Great news I sent you copy of my blood work
Please let me know if you got it .

My 4th Gen HIV Negative :)

However RNA test is not ready yet , but I think this is safely to assume
that it also will come back negative because since my condom broke
it has been over 2 months . Since then I had probably 8-10 more encounters
but I used condom with all of them . ( and luckily they did not brake )

anyway, so in regards to my blood work ,
can you tell me why I have high MPV level ?
What is it ? what can be causing ? and what should I do ?

Second , My HSV levels seem to be going down
Last time about 4 months ago when I tested they were around 5.1 both
doctor
Answered by Dr. Sunil Jangid (26 minutes later)
Brief Answer:
The blood report is not attached herewith to comment so send it again.

Detailed Answer:
Dear XXXXXXX

Welcome back to HCM.

4th generation test for HIV is negative after 2 months of exposure than you are absolutely safe and RNA PCR will also come negative so no need to worry.

You forget to attach the blood report so send it again. High MPV level basically means that your platelets are larger than normal which can be due to recently made new platelets by the body as a result of infection or medication.

As per definition young platelets are larger than mature platelets. As they start mature, the size shrinks and the MPV will likely to decrease. You don't worry too much about this lab result if the platelet count is normal.

If HSV level is decreasing than it is also good sign but to comment more better to send the blood report to me. Be relaxed and move ahead in life as you are risk free. Wishing you happy and healthy life.

Hi, I have provided some attachments. Please review them.

Answered by Dr. Sunil Jangid (4 hours later)
Brief Answer:
The reports are normal except for HSV 1 and 2.

Detailed Answer:
Dear XXXXXXX

All your reports are normal except HSV 1 & 2 which you had discussed earlier also. Take some antiviral medicines after consulting some dermatologist for herpes. Always use condom while having sex with your wife to prevent the transmission of infection.

The decreasing value for HSV is good sign for you so be relaxed and move ahead in life happily. Avoid excessive stress or tension because now you are 100% risk free. Wishing you happy and healthy life.
