Brief Answer:
needs investigated

Detailed Answer:
hi,
well actonel is known for side effects like muscle cramps, bone pain, vomiting, tingling, etc. However, the acuteness of neck pain and episodic nature of your sisters pain goes against the pain suggestive of due to actonel side effects. Although a possibility can't be ruled out.
On the other hand, osteoporosis does predispose to bone fracture, esp. on minor trauma such as fall while bathing, etc.
Pain of such intensity can often cause vomiting.
I suggest stop actonel until further advice from your treating consultant.
Get an XRay spine done, to rule out any pathology of fracture.
It needs to be done as an underlying diabetes can mask typical presentation in most cases.
