Question : I am a 34 year old male who was diagnosed with a anal fissure several months ago. I have been having a lot of problems with it for the past week; pain, burning, moist anal area, and a little bit of discharge. Is this normal? I have been taking fiber supplements and sitz baths for a few days-should I continue this or should I consult my regular doctor again?

Thank you for the query.

Anal fissure is a kind of wound. It appears due to hard stool, constipation, pushing hard on stool. Once the wound appears, strong pain during stool passing appears along with some trace of blood in the stool.
It is very important to have this wound treated property. Otherwise, it may turn into a chronic fissure (chronic fissure may need surgery).
Fiber supplements and sitz baths are fine. But may be not enough. You should definitely continue fiber supplements. Sitz baths should be with Oak Bark. You should also start suppositories containing hyalurinc acid. Such treatment should heal the wound in a week or so.
If after a week, there will be no improvement, you should consult proctologist. Nitroglycerin cream should be prescribed to relax anal muscle.

In a meanwhile you should avoid pushing hard on stool, lifting heavy objects.

Are the suppositories you recommended available over the counter? Also, would a cream form be available and as effective?

Yes, suppositories should be OTC available. Cream is Rx only. It is effective. And it can cause headaches as a side effects. It should be used if sitz baths and suppositories do not work.
