Suggest Treatment For Pain And Swelling Of Upper Front Tooth

Question: I had an accident when i was my13 year old and one of upper front teeth lost its half protion. After seven years I put a cap on it, without root canal, since the doctor who did the cap said that the root portion is healthy. now it is after 20 years i got pain in the both upper front tooths, it is sevioer in the capped one. I have consulted a doctor, he suggested , after xray root canal for the both teeths, also given to me EDOMOX-C for 4 days, Metrogyl 200mg for 3 days and combiflam for 3 days, for the time being. but after three days there is no considerable reduction in the pain and swelling. what shall i do to cure it

Thanks for your query, I have gone through your query.
As far as the pain and the swelling is concerned, it is because of the tooth infection in the capped tooth. The tooth loses its life following trauma, it may take many years and later it can get infected. So the severe pain and swelling is because of the pressure(pus formation or abscess) within the pulp chamber of capped tooth. So the medicines what you are taking is fine but the cause has to be removed. So consult a endodontist and get the access cavity opened and allow the pus to drain. Once the pus drains the pain reduces automatically. later get the tooth treated with root canal treatment.
For the severe pain you can take analgesics like ketorol along with the other medicines you are taking now.

Now the pain almost reduced but i feel pain inside the bottom of my left nose. Can I get the access cavity opened and allow the pus to drain without removing the existing cap. What is the procedure for drain out the pus under this situation? The Doctor whom I consulted recommended to make root canal treatment without removing the existing cap, ie. by drilling through the existing cap. Would this able to drain out the pus completley or is it a healthy practice to me. Is there any difficulty to removing the existing cap, can we use the existing cap again since it is too costly and full of ceramic and still in good condition.

Thanks for your feedback. The main purpose of access cavity preparation to allow the pus to drain. If pus is not drained, it will create lot of pressure within the bone near the root tip. Once the access cavity is opened the pus will drain and infection will come down.
Regarding the ceramic crown, we can do the root canal treatment by drilling a hole through the cap itself. Do not get the cap removed, you can go ahead with the root canal treatment by retaining the same cap.
