Suggest Treatment For Persistent Fever Despite Medication

Question: Dear Sir,
From last 10-15 days my wife is suffering from high fever at initial stage doctors identified that it is typhoid accordingly they started the treatment for the same.
As long as the antibiotic is in her body the fever goes down and again the fever goes up. she is also suffering from omitting and she is not able to take any food. Doctor has also identified that, their is a swelling on her intestine because of regular omiting
what will be the exact reason.
Please advise.

I appreciate your concern for your wife.
Long standing fever warrants detailed evaluation by physical examination as well as lab investigations.

To better assess her condition, I would require further information like
Is this fever is associated with chills or rigors
is it intermittent or continuous fever ( I mean fever comes every day)
Any history of burning urination or increased frequency of urine or dark colored urine
How's her appetite
any complaint of pain in abdomen or loose motions

Please upload me all the reports her doctor has advised

Swelling on her intestine can be related to inflammation of lining of the intestine, which can be controlled by giving rest to the intestine and symptomatic treatment.
Meanwhile please follow these guidelines
Ask her to have fresh fruits, vegetables and good balanced food
You can give her acetaminophen tablets for fever with pantoprazole or omeprazole and antacids

Antiemetic drugs like domperidone or ondensatron can be given to control her vomiting.
Multivitamin supplements are also advisable.
Please revert back to me with all these details so that I will be able to tell you more accurately

Dear Sir,

Thanks your response. Please refer to below points.

1. Fever is intermittent. as long as she takes tablet or the dose as prescribed by doctor fever will not be their. after 10-12 Hrs fever comes back again and that to high fever.
2. two days back she was having stomach pain but now it is not that much still little bit pain she has.
3. One week days back she was having stomach pain and loose motion, followed by vomiting but at present No vomiting and loose motion
4. Presently I am not with my wife she stays 500 Km from here. Now a day she is at her native place. because of the weakness she is unable to travel to XXXXXXX

I will try to collect her reports. But the doctor has intimated that she is having Typhoid.At Present she is not able to take any food. Once she takes any food (fruit, fruit juice or coconut water) she starts vomiting and loose motion.

Kindly advise what you feel.

Looking at your short description only and without physical examination and lab investigations, It will not be possible for me to tell you exactly what could be the diagnosis.

You can ask your wife or her doctor for a scan copy of investigations or a photograph of the reports as well as doctors prescription and upload me the same.

You need not to worry she will be alright very soon.
Please consider anti emetic drug with anti acidity medicine and multivitamin as I have advised previous reply.
I am sure she will feel better with these drugs apart from antibiotics or other drugs which her doctor has advised.

At present her Hb is 6 meantime I am trying to collect other lab reports also and update you the same.

Her hemoglobin is very low

Hemoglobin 6 is very low if it decreases further then she may require blood transfusion.

I would advise you to look after her in better way if her condition is not improving.

Ask her doctor for the reason behind low hemoglobin, whether she has low hemoglobin prior to fever or it has reduced due to present illness

She may require hospital admission and supervised treatment if she has no improvement in her health.
