"Brief Answer:
Viral fever, symptomatic treatment

Detailed Answer:
Hello, greetings!
Thanks for posing your query in hcm.
It looks like your brother is suffering from an episode of viral fever.
You may give him Ibuprofen+ Paracetamol tablet combination thrice a day for the fever and bodyache.
Also, he may take over the counter antihistaminics like tablet Fexofenadine 120 mg twice a day after food for 3 days.
He may go for betadine gargles to be done twice a day also, salt water gargling in the night time.
Let him have plenty of fluids to keep himself well hydrated.
Viral fever usually resolves on its own in around a few days and antibiotics are not needed.
If fever persists for over 4-5 days, then he may get a blood test and throat swab culture test done.
If you have any further doubts/ clarifications, kindly get back to me."

My brother is taking paracetamol and meftal, along with zifi, but still fever not coming down from 101, an at night it was till 105, we visited doctor nearby, he treated with injection. Fever was reduced but still it is not coming down from 101 or 102, even after treating cold water dipped with cloth.

Brief Answer:
Wait and watch.

Detailed Answer:
Welcome To HCM. I am Dr Anshul Varshney. I understand your concern.
Viral fever is high grade in initial first 3 days and it even doesn't responds many times to anti pyretic drugs.
My recommendations in such case would be to give him Paracetamol 500 mg every 6 hourly and in case he gets high grade fever in between he may take Ibuprofen.
You should do tepid sponging on his whole body to reduce fever.
You have fight with the fever initially like this only for 3 days. After 3 days his fever would improve but still it would be coming for total 5 days.
As a general protocol he should be investigated if fever persists for more then 5 days.
Keep patience. Make sure he drinks plenty of water or Oral rehydration solution to prevent dehydration.
I hope it helps you.
If you have any further query, please ask me.
Wish your brother a speedy recovery.
Take Care.
