Suggest Treatment For Pus Discharge In Ear While Having Eardrum Perforation

Question: Hello Sir, I am 44 years old, 5.3' Height, 65Kg Weight. 20 days back, a puss form has come out from my left ear. Immediately I met the ENT doctor and they have shown in video that there is a small pin size hole in left ear and advised me to undergo surgery. But I have not gone through the surgery so far. Since 20 days I am feeling like a air blockage in both ears. But I can hear from both the ears. Musical instrument sound, vehicle sound irritates me a lot now . Please advise.

1. A pin point perforation implies a ventilation failure due to Eustachian Tube Dysfunction ( ETD).

2. The pus is formed due to stagnation of secretions. The middle ear (behind The eardrum) is always wet due normal secretions. These get infected.

3. You should get a PTA ( Pure Tone Audiometry) done to check the hearing levels.

4. Which ear drops were you using?

5. The audiometry tests will reveal why the ears feel blocked and treatment can be started.

6. Wait for a few weeks before planning any surgery.

I am using Drep ear drops. I have XXXXXXX also.

The XXXXXXX levels are:

Left Ear: 33 db
Right Ear: 15db

1. Check the contents of the Drep ear drops. If they contain Gentamicin, discontinue the drops. If not, you may continue using them.

2. The higher hearing loss in the left ear signifies pus collection in the middle ear if the hearing loss is conductive (and not a sensorineural hearing loss). Upload an image of the PTA report.

3. An ear pus C/S (Culture & Sensitivity test) will help in deciding any further medication.
