Suggest Treatment For Recurrent Folliculitis On Shoulder

Question: Doc, u were advising me on my recurrent folliculitis on shoulder and upper back... U had suggested R cin 650 and augmentin 650... And pus culture test for active lesions with pus.

At that moment I had no lesion with pus but over last two days I have developed couple lesions with swollen bump and pointed pus peak... Rest of the lesions are on decline and are healing well.

Unfortunately am not in XXXXXXX now and based in Lagos Nigeria where may not get reliable pus culture test...look forward to your advice whether to start cause of r fin and augmentin for 1 week as suggested by you earlier.

I had been evaluated as hsv1 positive with igg of 1.93 Against XXXXXXX of 1.2, this was in Nov 15 and had taken one 10 day course of Herparex 400, do I need to repeat the same or not.

Hi. Thank you for writing to us

I have noted your concern and I am pleased to help you regarding that.

I don't think there is a need to repeat Rifampicin so soon (at least for 3 months after last course). It is not advisable to repeat it frequently as bacteria may become resistant to it, if used repeatedly. You may try to procure a topical benzoyl peroxide 5% gel and use it once or twice daily for the new lesions. Also you may try to procure a benzoyl peroxide body wash, and use it as you would use a soap, daily, at the time of bath.

Thanks doc, will not take r cin, can this be another outbreak is hsv1 lesions... Shall I take 5 days of Herparex.... Or only body was will do...

My family is coming to Nigeria later this week, if they have to buy this body wash, by which name they can buy from medicine shop and in which quantity as I won't be coming back to XXXXXXX for a year at least.

Hi doctor, am unable to find here benzoyl peroxide gel or body wash in concentration u suggested, though am getting benzoyl peroxide cream in 10% concentration. Shall I buy the same.

Also my lesions on left shoulder have multiplied from 1 on Sunday to 4 now. I also have developed 1 lesion each on my left buttock and left thigh. Important to share that had safe Swedish oil massage on Sunday and post that lesions have started to increase. Did massage cause some issue here...

My other worry due to my hsv1 igg coming positive at 1.93 is that this may be a hsv1 outbreak, I had potentially my first in sept with lesions on shoulders and upper back only which started disappearing after 10 days of Herparex 400 followed by 10 days of r cin 600. My question is shall I repeat 5 day course of Herparex 400, will that create any side effect if I don't have side effects. I just want to get rid of these lesions or folliculitis which are on my shoulder and upper back since September. Pl advice.

Also somewhere I read that igg 1.93 is low positive and maybe false reading as well, so will you suggest I repeat blood test for Hsv again.

In between post having Herparex and , I had probable urticaria wherein I was taking Allegra 180 every day for 15 days to manage itching and redness of skin at various parts of my body. However for last 5 days this urticaria has reduced significantly while still I feel itchiness on various parts of my body but it is manageable and I'm managing without Allegra.

Finally am attaching photographs of my left shoulder, left buttock and just for comparison photo of my right shoulder with healed lesions.

Look forward to your advice. Sorry for long mail.

Hi.

I have reviewed the Images. Oil massage can certainly increase Or flare folliculitis/ acneform lesions and therefore I would advice you to avoid that in future.
Regarding benzoyl peroxide, I suggested you to use benzoyl peroxide gel/ cream. I suggested a preparation i.e Brevoxyl cream and Brevoxyl 4% creamy wash. These would be available only in XXXXXXX and are marketed by XXXXXXX pharma. If you can find these perfect and good Or else you may use benzoyl peroxide 10% cream.
These lesions have nothing to do with herpes. Herpes does not present at back or shoulders. Hsv usually presents with oro-labial Or genital lesions. Therefore no need to repeat herperax. Though you may take an Oral antibiotic e.g Doxycycline 100 mg twice daily for 2 weeks, however, not R-cin.

Thanks, have taken doxycycline for 2 months between oct and Nov and it was not helping much... You still will suggest I go with doxycycline.

Also I was wanting to repeat Hsv test as I read that low positive igg can be wrong test result as well..shall I repeat the same.To add, I was also given Minolux 100 in place of Doxycycline by the skin doctor in XXXXXXX alongwith Herparex 400 and that calmed the lesions down...

So instead of Doxycycline, shall I take minolux 100 for next 2 weeks...

Minocycline (Minolux) would be fine. I don't think these skin lesions are anything to do with herpes, though you may repeat the test, if you wish.
A positive IgG above the index value of 1.10 is a positve result. If it were in 0.90-1.10 range, then it is considered equivocal and worth repeating.

Thanks Doc, will start with minolux then... for 2 weeks? how many times I take it in a day?

Also if you can share what can be the probable cause of recurrent folliculitis which is literally not going since middle of Sept'15.... am worried if this is due to any major reason...

My vitamin D and vitamin B12 are low... getting injections for them for last two weeks.

My TSH is high at 7.2 while T3 and T4 are normal...

Havnt done diabetic check... yet...

anything else need to be worried....
Sorry am troubling you, but need the worry to be away from me, doctors here in Lagos did half kill me with various severe options. And my mind wondered on Internet and read various sites which further drowned me...emotionally.

My key thing is how I can protect my family from hsv1, are there any tips you can share with me...

Also how did I recognise Hsv 1 outbreak, any visible symptoms which can help me identify and take appropriate precautions.

Looking for your advice.

Lastly I was feeling itchy in my groin area, did check now, did'nt found much. Still forwarding the picture if you can assess as well.

I think my problem is equally psychological as clinical... Will look to your advice how I approach the same.

Hi.

Minocycline is to be taken 100 mg once daily.
I am of the opinion that you most likely have acneform eruptions/ acne on shoulders and back. This is not a serious condition. In fact it is quite common in dermatological practice. Vitamin D, TSH are not related to this however you must take appropriate treatment to correct those.
Hsv 1 usually presents as sores (cold sores) or grouped vesicles/ erosions either on lips or perioral region, not on shoulders of back. Rarely Hsv 1 may present as genital sores.
You have a mild fungal infection in groins. I suggest you to use a clotrimazole 1% + beclomethasone dipropionate 0.025% cream, twice daily for a few days. Along with this you may take a tablet of cetrizine 10 mg once daily for a few days

Hsv does not transmit by casual contact

Detailed Answer:
Hi.

Hsv does no spread through casual contact like touching or sharing items. Hsv only gets transmitted through close contact i.e kissing, sexual contact etc. Moreover, the risk of transmission through direct contact is more during an active episode of hsv 1 e.g an episode of cold sores or fever blisters, than at other times. Therefore, unless you notice any blisters or sores, on or around the lips, you don't need to worry about transmitting it to your family members. Only at times of active episode of cold sores or sores on lips, you should refrain from direct contact i.e kissing.
