Question: I am 68. Have been prescribed - Urimax 0.4mg (Tamsulosin Hydrochloride) for BPH. How do I recover from retrograde ejaculation ? I get zero ejaculation after starting this medication since a year ago with gaps of two / three months.

I evaluated your query thoroughly .
* Retrograde ejaculation is due to the effect of Urimax ( via alpha receptor
blocking mechanism ) on sphincter of the bladder which controls closure of
sphincter during ejaculation ; occur with less no. of patients consuming
Tamsulosin you are one of them .
* Recovery comes regarding ejaculation once the Tamsulosin has stopped & all
residual effects of it from the blood stream wears off .
- Certain drug molecules are there for retrograde ejaculations which are
caused by nerve damage / other issues but they have their own side effects
& you don`t require them at all .
* Regarding starting other molecule for BPH you must consult your Urologist .

I did have a mild slowing of urine stream before I started using Tamsulosin. But had no significant improvement with the medication except ended up with zero ejaculation. Q : If I stop taking Tamsulosin from today, approximately how long will it take for the residual effects to disappear ?

Hello XXXX,

* Firstly it is retrograde ejaculation, the semen coming outside from urethra is
going back inside bladder so actually, medically it is not zero ejaculation. Yes, you can consider this as zero external ejaculation.

* Usually, it does not affect the sex drive or arousal, so if you are satisfied with that, you
can continue Tamsulosin, but if at all you want external ejaculation of the semen, in that case, you have to discontinue the same.

* After you stop Tamsulosin, average time of 7-10 days will be required for the
residual effects to disappear.

I did stop taking Tamsulosin once for about two or three months but did not notice any external ejaculation. Maybe a slight late wetness which was insignificant. Shall stop taking the medication now and observe if it improves external ejaculation

Hello Dear,

* If the cause of retrograde ejaculation is 100% Tamsulosin then it is going to
revert it else there are neuronal reasons as diabetes / multiple sclerosis /
spinal cord lesions & many more in which case even after stopping Tamsulosin
it will not improve.
* At Present, you can stop taking the medicine now and see for minimum 2 weeks.
Later if retrograde ejaculation continues, then it requires further detailed urological
work ups.

* Will appreciate any further details which you want to share like any history of
trauma / surgery over back / other systemic ailments / any procedure over
bladder in past etc.

* When had you noticed full normal external ejaculation at last, whether at
that time was on Tamsulosin or not? kindly provide these details.
