Suggest Treatment For Severe GERD And Anxiety While On Xanax

Question: I have been having multiple hard spasms in my upper abdomen about the past three months that quickly take my breath away. I have been diagnosed with GERD and anxiety from PTSD. I am currently prescribed xanax .25 mg twice a day, pantoprazole 40 mg once a day and paxil 60 mg once a day. The spasms or hard hits as i call it seam to get worse after i eat and i also feel full and bloated, sometimes i get a little light headed and dizzy if i am doing something work related. The spasms can last from 10 minutes to all day off and on.

I have gone very carefully through your query. Your problem seems to be due to gastritis or gastroesophageal reflux as already diagnosed earlier. Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. If somehow this movement gets hampered, food along with the digestive juices would stagnate, stomach acid will reflux into the food pipe and on the other hand the bowel habits would not be proper. Degradation of undigested food in the intestines by the intestinal bacteria would lead to the excess gas formation and may lead to bloating.

In order to relieve yourself, you need to do the following:
* Avoid spicy food. Take small but frequent meals. Avoid long gaps between meals.
* Avoid alcohol intake (if it a part of your habit).
* Take Prilosec (omeprazole) once daily before breakfast.
* Take plenty of green leafy vegetables as well as other sources of dietary fibers. Take fiber supplements like isabgol/psyllium husk or senna. I would insist you to increase your dietary fiber intake.
* Regular morning walk and exercise to keep the bowel movements proper.
* If I had a patient like you I would perhaps have suggested Prodep 20 mg tablet once daily for at least 2 months (it may take time for the action to set in).
* Take curd or yogurt on daily basis. It contains beneficial bacteria and should help.
* Yoga and meditation should help.

With that being said is it common to have the hard spasms or palpatations frequently that take my breath away? Is pantoprazole what i should be taking? I tried nexium with the pantoprazole about a month back and it really made the spasms more frequent so i stopped the nexium. The spasms really get my anxiety up and i know that does not help the situation. I am a very active working man and between the bloating, dizziness, spasms, tiredness i am not sure which direction to take. Thank you for the information.

Yes, spasms are often felt and indeed they can take your breath away. Pantoprazole and omeprazole are similar and taking any one of these should be fine. Nexium is esomeprazole, a variant of omeprazole. It does not need to be combined with pantoprazole. Since the spasms are still occurring, I would insist you to take decolic tablet when such a thing occurs. Try taking Mucaine Gel (a liquid antacid along with anaesthetic) and it should provide symptomatic relief.

Try and follow the other suggestions given.
