Suggest Treatment For Stomach Pain

Question: Hello.. I've been suffering from stomach pain since 3 months. I have a throbbing warm sensation on my solar plexus area and gastric issue. I also have trouble eating foods without suffering from stomach pain even though I'm consuming only plain boiled foods with minimal sunflower oil and no turmeric, onion or spices. After taking medicines from a doctor post blood tests and full abdomen Ultra Sonography test, I consulted a doctor on this website who figured I'm suffering from Jaundice and suggested me only Omeprazole or Pantropazole once in the morning and Liv 52. I retook my tests with no improvement in my Bilirubin levels. Currently I'm taking only Sorbiline liver tonic and Omeprazole 20mg Once in the morning. Please guide me and help me heal faster. I have attached prescription and reports. Thanks..

Thanks for your query, information and reports.

You main concern is pain in abdomen since 3 months, throbbing warm sensation on solar plexus and gastric issue.

Please give me the detailed history related to your troubles:
- Is the pain for all the time or only after food?
- Is your motion normal or not- loose stool or hard stools/ constipation?
- Do you get undigested food in the stool?
- Any urinary complaints?
- Nausea or vomiting?
- Does the pain increase immediately after food or after 1 -2 hours after food?
- Any bloating/ distension/ gases in the abdomen?
- Any reference to the back or shoulder?
- Since you are consuming only boiled food the way you explained - has the pain reduced or not?

I would advise you to give answers to my all the questions, please.
I think you need further tests like:
- Upper GI Endoscopy.
- CT scan of the abdomen.
These are necessary to find the cause of your pain/ throbbing warm sensation on solar plexus are.

Mild increase in bilirubin and SGPT alone can not cause such a problem for such a long time. Hence the need to take an opinion of a Gastroenterologist for clinical evaluation, actual examination of the abdomen and further examination/tests so that the cause can be found.
Unless the proper diagnosis is made, proper and effective treatment is not possible.

I've answered your questions:

- Is the pain for all the time or only after food?
I feel a soreness after waking up from sleep in the mornings or in the evenings as well and the pain lingers throughout the day. It doesn't happen everyday and is generallt triggered after I eat food with bit more oil content.

- Is your motion normal or not- loose stool or hard stools/ constipation?
The motion is usually normal. Sometimes it gets bit hard but doesn't cause any trouble. Rarely after breakfast I feel warm in my stomach and I get loose stools but it happened only once in the past month.

- Do you get undigested food in the stool?
No.

- Any urinary complaints?
Every morning my urine has a distinct yellow colour but it doesn't appear such if I consume water afterwards or if I increase my water intake before sleeping at night.

- Nausea or vomiting?
No. I do feel bit giddy after getting up from bed if I lie/sit for extended period of time.

- Does the pain increase immediately after food or after 1 -2 hours after food?
The pain is generally felt after 2-3 hours or more after consumption of meals. Mostly after waking up from sleep.

- Any bloating/ distension/ gases in the abdomen?
Yes. I have bloating and some distention in my stomach. I feel better after I pass gas or after burping or else the pain lingers for a long time. It is not easy for me to pass gas or burp. The gases seem to travel within my stomach and stay there.

- Any reference to the back or shoulder?
Bit of soreness on my shoulder probably because of too much rest and lack of enough physical activity.

- Since you are consuming only boiled food the way you explained - has the pain reduced or not?
The intensity of the pain has reduced. I'm also taking digestive enzyme like Digiplex syrup after meals twice daily. I'm also abstaining from eating non veg. But the pain is still there and slowly subsides after burping but leaves a soreness sensation afterwards.

Thanks for your elucidate feedback.
From this detailed history and the findings of the reports and your history I am thinking that you may have some problems related to the stomach or duodenum and sphincter of Oddi which is at the end of the combines bile and pancreatic duct which controls its release into the duodenum.

In addition to the Upper GI endoscopy and CT scan of the abdomen I would strongly suggest you to undergo the tests of:
HIDA scan if available or ERCP (Gastroenterologist does this) as a diagnostic as well as therapeutic in some cases, as there is spincterotomy and stenting if required.

All the tests will help us to get a proper diagnosis and possibly the reason of mild derangement of the liver function tests and your symptoms.
