Question: Hi doctor I have slightly swollen ankles the right one is more swollen than the left one only noticed them last night when I was sitting down my right leg felt like it had a bit of an ache in it while travelling home on a 30 mile journey yesterday evening I was a passenger I'm a female aged 51 I also am the same person that emailed yesterday about the pea size lump beside my navel are the two connected I'm also not sleeping well at night only this last few weeks i only sleep for about 4 hours I go to bed at 12 then I'm awake from 4.30 on is all of this connected I have not been with the dr yet regards XXXXXXX

This is highly suggestive of ‘dependent edema’ – swelling on prolonged standing or sitting / reduces or disappears on lying down. It is generally due to weakness of vessel (vein) wall / incompetent valves.
However, because it is the first time you have noticed, and is with ache – you should see you Primary care Provider (PCP) or ER, to exclude thrombosis (clot in calf veins) … blood test and Doppler study are needed to rule it out

The nodule near the navel is unlikely to be related to it. That needs detailed examination. Any swelling must be seen by a surgeon – if inconclusive, biopsy is a must. Especially around the umbilicus, because the area of lymph drainage is not only from abdomen but also from axilla to groin!

Appropriate treatment demands accurate diagnosis / And that demands examination; probably investigation too; GET EXAMINED
The treating doctor alone could conclude better based on total clinical picture…
