Suggest Treatment For Symptoms Of GERD

My story is very long, irritating and damaging my life.
Started in Ramdan 2014, actually from more than 5-6 months. one day, on iftar time and after the first piece of food I felt like i need to cough and there is something like phlegum. 1 month after phlegum started to increase and when i sleep i felt it more. Than my voice started to have same hoarseness, phlegum increased when i decided to go to an ENT because I also had past allegric throat stories. He gave me some antibiotics and phlegum medecines and throught these 10 days my case was worst caughing 24 hours and cant sleep. went to other ENT who said acid reflux and told me go with nexium for 3 weeks and come again to see with some allegry medecines for my nose he said that it is responsible for phlegum. After 3 weeks I improved by 2.5% I think not more not less. then after these 3 weeks he told me lets try and continue and after 1 week I improved significantally with less phlegum until i felt mild chest pain while playing football. In these period i started also feeling something stuck in my throat, splits of liquid going up to the throat at night. Chest pain increase, liquid started to come even while i am eating with the phlegum also and constant throat clearing. I went again and he told me continue with only nexium 40mg. Days proceedeed with me having more and more chest pain that I started to read on the internet that decreasing food can help. In 2 weeks i only eat boiled food, low fat food an no improvement. My case was worsening day by a day until i decided to visit gerontologist who told me we will increase necium to 80mg, take motilium and if no improvement after 3 weeks lets go to the endoscopy. No improvement happened and foood started to stuck not only in throat but chest and from this i started eating only 1 or 2 meals a day because of this pain. Did the endoscopy after 3 weeks and nothing appeared unless H pylori doing mild gastritis !! continued on the medecine with all this irritating symptopms of bloating, food stucking in trhoat and chest, abdominal pain, chest pain, phlegum, feeling water always in the stomach. Visited 2 other gastroenterlogists gave me other medicnes made manaometry and nothing apeared chest x ray and nothing appeared ! and now on nexium 40mg morning 40mg evening and gast-reg for motility disorders and aerius and maalox. for the last 2 months also no improvement symptomps ranging from severe to very very very severe and i have no hope now really for a 6 months ( 4 of medicnes ) of irritating life damaging symptoms ( last 3 months ( boiled food, no fats, no caffaine chocolate etc,,, elevating bed,, sleeping 3 hours after eating) !!
Option of endoscopy can be discussed

hi,
from your symptoms of bloating, heartburn, sensation of food stuck in throat, it appears strongly to be a condition of Gastro Esophageal reflux disorder( GERD).
A condition due to dysfunction of lower Esophageal sphincter, that cause reflux of acid containing food to reflux back into esophagus, which irritation causes these symptoms.
a 24 hour Eaophageal PH monitering is confirmatory investigation done.
Treatment can be by having certain lifestyle modifications to prevent it like
weight reduction,
avoidance of tea caffeine, spicy food, smoking, heavy meals
Medication treatment- Antacid drugs like proton pump inhibitors in twice a day dosing can provide symptomatic relief.
Surgery- in severe cases, one needs to examine and asses severity of dysfunction before. plication of the Esophageal lower part is done.
in your case the prescriptionnof nexium is appropriate, however one needs to asses severity as in severe cases nexium won't be much effective.
Ph monitering and endoscopy are two investigatory option you can discuss with the consultant.

the problem is that the problem is even without eating as when i wake up when i do any thinkg.
There are no other possibilities other than acid reflux ?

I take your problem of having the symptoms even when not eating. It could be explained due to anxiety, that is commonly known to aggravate such symptoms, and are commonly associated with reflux disorders.
Look out for symptoms such as continuous apprehensive feelings, stress, sleep problems, these could be associated problems suggestive of anxiety.
Your symptom profile is very strongly suggestive of acid reflux disorder.
Along with the previous medications, addition of a long acting low dose benzodiazepines can be discussed with your treating doctor.
Therefore an underlying anxiety problem can be probed for.

Hi,
Well, anxiety may have no direct causation to stomach ache and phlegm, however it can increase tendencies for acid disorders.
Besides, the symptoms of stomach could possibly be explained by GERD. There is also a possibility that these problems exist independently.
As the done investigations have been found normal, the likelihood of any organic pathology is less.
If some one with similar symptom profile comes to my clinic, I would advice him to see a psychiatrist, as its imp to see for functional cause.
