Question: Hi, I had heavy cold 3 weeks back while traveling. Then I landed back home after long flight. I have tinnitus developed. I did my audiogram and consulted an ENT phycian. He presribed some antibioitics and anti allergy tablets. Mlilxim 200 mg, XYZAL and Goonj Forte. What should be homely remedy in addition ? The problem persist in my left ear and I am due to fly again in 1 week.

Thank you for your query.

1. This is a common problem if you fly during a cold., known as Ear Barotrauma. Avoid the next flight till you are better.

2. Get an Impedance Audiometry (Tympanometry) done and upload the graph here.

3. You can use OTC Nasal decongestant drops (containing 0.1% Xylometazoline) for a few days (but not more than two weeks), provided you are not hypertensive or have glaucoma. Carefully allow the drops to descend along the outer walls of the nose. The tinnitus should come down after treatment.

4. If you cannot avoid the next flight, use these nasal drops (after steam inhalation) an hour before the flight.

Dear Dr. XXXXXXX Bhatti,

Thank you, I appreciate your response. I have focused on steam inhalation and I am using the nasal drop too. The intensity of sound increases after bath. The hissing sound is disturbing in my work too. Please advise if B12 injections and Zinc tablets could help.

Per my Physician's advice, I am now on Tinnex 2-0-2 daily for 2 weeks now. No change in condition though. But the Tinnex manufacturers say, it effects only after 3 months of continuous consumption.

Please guide one more and final time.

Hi,

Thank you for writing back.

1. There is no reference as to your age or occupation. It is possible that your attention was drawn to a pre-existing tinnitus by this incident.

2. Given your history, your tinnitus is more likely to be temporary and due to changes in middle ear pressure (hence the importance of the Impedance Audiogram). This would also explain why the tinnitus changes after a bath.

3. Tinnex (caroverine) is designed to work in a type of tinnitus known a cochlear synaptic tinnitus. The ideal way to use this is by a single intravenous dose over an hour followed by a maintenance dose of (20 mg x 4) capsules per day for a minimum of 2 months.

4. Since your history is only of 3 weeks and your Impedance Audiogram has not been done yet, it is too early to start Tinnex. Take B12 and Zinc tablets instead of injections.
