Question: Hi there i hope you can help. I have some very abnormal bleeding including some itchiness around my vagina. The bleeding started 3 weeks ago and began as spotting. I take the contraceptive pill and had taken it back to back however after a week of constant bleeding i stopped, this is when i had an extremely heavy bleed which was uncontrollable meaning i had to restart the pill. This eased bleeding for 1 day and now i only bleed shortly after taking it. I dont bleed all day then when i take the pill i have an extremely heavy bleed for 2-4 hours?

Brief Answer:
Some women have breakthrough bleeding when taking the pill back to back

Detailed Answer:
Hello, and I hope I can help you today.

Taking a second pack of pills without a break in between can be used safely to delay your period, but it doesn't always work, especially if you have been on the pill for less than three months or for many years. Once you stop the pill you will have a normal period- which may be heavy if you are on a very low-dose pill. Suppressing your period back-to-back works best if you are on a monophasic pill (meaning the same hormone dose all month) and not an ultra-low dose one... (with 20 micrograms of estrogen or less).

If you haven't done so yet- I would not take any more active pills for one week straight... You may bleed during that time but it takes that long for your uterine lining to regrow and it will slow down on its own. Then restart the pill... and take it every day even if you bleed a little. This should reset your cycle back to a monthly one by the next pack after that. Continue on a monthly cycle and see what happens. If you continue to breakthrough bleed, you should visit an OBGYN so he/she can do an evaluation. There are uterine problems that can cause irregular bleeding, like polyps and fibroids, especially if you're over 35, and also you may benefit from a different pill choice if you like to try to skip your period. There are specific birth control brands that are designed to do this.

Regarding your itching, I am sure this is due to being wet from the constant bleeding and the pads or liners you use. The brand "Always" of pads and liners frequently cause irritation as many people are allergic to the dry-weave coating. A topical antifungal cream, like the ones used to treat yeast infections, can be applied externally twice a day to the itchy areas and that should get better within a week. And use another brand of personal hygiene products if you use Always brand.

I hope that I adequately answered your question and that my advice was helpful. If you need any clarification or have any further questions please do not hesitate to contact me.

Best wishes, Dr. Brown
