Question : Hi doctor.. I have been developing fissures in the foreskin and it's very painful and had taken some anti fungal pill after which it went away only to return. I masterbate almost every second day using my spit as a lubricant....now it hurts a lot when I retract my foreskin... Please help me

Brief Answer: Gentle retraction, antifungal cream sparingly, tab Detailed Answer: Hi there. I hear you & your condition suggests to me that you ail from a condition called candidal balanoposthitis. While this may be a fungal/yeast infection, it is very often admixed with a bacterial infection. Have you checked your Diabetes status as you are pushing 40 & if there is a strong family history, please do check as it is a common occurrence in diabetic individuals albeit not exclusive to them. Also remember to check your partner/spouse & treat them too even if the infection is not always obvious in them! Gently retract your foreskin & use some mild lukewarm water to wash it three times a day. Do NOT apply too much of the cream I suggest as don't want it to 'cake' & let the skin breathe. Please use a cream called Candid-B locally for 2-3 days & stop. Then switch to a cream called Nizoral applied twice daily for a further 7 days. In addition, there is a kit called FAS-3-Kit which contains 3 medications to be taken as a single dose over ONE day (breakfast/lunch /dinner) & both partners need take it as a single dose. This may need a prescription & if near Bangalore, you are most welcome to XXXXXXX hospital for the same & further advice or please do source a dermatologist nearest you using this HCM website.
