What Are Klabeissela Bacterial Infection Symptoms?

Question: I was diagnosed with UTI in Nov. 14. I think I had it from before because I had urinary problems which were being overlooked. In Nov. I had high fever which was not subsiding despite high dose of broad spectrum antibiotics. There was a urine test and a culture and sensitivity test. The culture test showed Klabeissela bacteria. The physician initially put me on high dose of Augmentin for almost three weeks and after the fever was gone he asked me to stop treatment even though the urine test showed pus cells. I consulted a urologist who advised me that UTI should be cured completely as it may later on damage the Kidney. For a month he gave me antibiotics and told me to get a urine culture 8 days after completing the treatment. The second urine culture showed E-coli bacteria. I was again put on high doses of antibiotic for 60 days. the period ended on 27th. feb. I got another urine culture on 12th. march and it growth of e-coli while the urine test showed 15-20 pus cells. I have been given 1gm. of Amicin inj. OD for 7 days and again put on antibiotics- GMD-LF (HS), CRANPAC-D(BD), ALFUSIN (OD), REBEMIN-20(OD). I am 49 yrs.old, 5ft.8' and weigh 90kg. I dont have diabetes but have High BP. The blood test shows S.Kertenin as 1.2. Please tell me why even after 4 months of continuous treatment it is not cured and for how long will this treatment continue? What are the chances of kidney damage due to this infection? what are the causes (i never had sex outside marital relationship)? Have I to live with it?

Thanks for your query,based on the facts that you have posted it appears that you have Chronic UTI since may be from years .

I shall be happy if you can post me few more details to help me in making comments on your problem.

1) Do you have obstructive voiding symptoms like frequency for urination ,poor stream,nocturia etc?
2) Have you done ultrasound Scanning of abdomen and pelvis ? If yes what was the residual urine volume?

Chronic UTI is very embarrassing for patient and treating physician as well.

The bacteria change their sensitivity pattern frequently hence they do not respond to antibiotics to which they were sensitive earlier.

One has to do Urine culture at frequent intervals and take the appropriate antibiotics as per culture report till the infection is eradicated completely.

Please consult Urologist again and get endoscopic examination of bladder(Cystoscopy) done to rule out any gross pathology in the bladder or bladder neck.

Take long term course of Nitrofurantoin twice daily as a prophylaxis.

Since your serum creatinine levels are normal you need not worry about the damage to the kidneys.

Thank you Doc. for the answer. So far as your queries are concerned I am sending the reply. So far as nocturia is concerned I have to wake up in the night once to urinate only when i drink water just before sleeping which I do as I am on medication of High BP and I have been told that drinking water just before sleeping prevents attacks in the morning. Further, I take my dinner just before sleep. If for some reason I have to remain awake for an hour after dinner I urinate before sleep and then I dont have to wake up in the night to urinate- but then in the morning the bladder is full and the first thing I have to do is to go to the bathroom and urinate. For almost a year I had poor stream but i always thought it was either due to being overweight or may be physically inactive life. I am an advocate by profession. So far as the frequency of urination is concerned it has been there since long- may be 5-6 yrs.(I dont remember) but then again I thought it may be because I drink a lot of water (sometimes 4 litres a day) and had frequent cups of coffees and teas. Yes, I had an ultrasound of abdomen and pelvis and the report shows that no calculus or hydronephrosis changes are present and no ascites or lymphadenopathy was seen though the left kidney shows a cyst (72 mm) at upper pole- in 2007 also there was a ultra sound which showed it to be a anechoic cyst and the docs. advised me that it is not serious and does not require surgery or medication. The report regarding Urinary bladder shows no calculus or debris present, Full bladder volume:- 229 ml, and PVR-NIL. The prostate shows to be normal in size, shape and echopattern and capsule intact. Size- 35*43*31 mm. and weight-25 gm. The impression recorded in the report is that moderate fatty changes in the liver, left renal cyst at upper pole, and Grade-1 prostatomegaly which docs.again say is not an abnormality. I hope that it answers yours queries. How do I prevent further UTI and for how long should I continue with the antibiotics treatment before going for a cystoscopy which my urologist has not yet advised me.

Thanks for feedback.

As per report of ultrasound scanning you do not have obstructive voiding symptoms
and you do not have residual urine which is common cause of getting UTI at this at this age .

However Cystoscopic examination will help to know the cause of getting repeated UTI .

Please repeat your urine culture again and take appropriate antibiotics for 3 weeks .and continue taking Nitrofurantoin as a prophylaxis till you become symptom free .
