Question: Hello

Last week I had fully protected intercourse with a stripper in a strip club.

The stripper also rubbed my penis against the outside of her underwear and also the inside of her underwear, my penis did not have any protection when the latter was done - there was no contact with the vagina at all at this stage. I am suffering from anxiety that I may have caught HIV would this be the case or any other STI because I rubbed the underwear with my penis it may have secretions would this cause me STI's i.e. HIV my penis has a slight itch from the front but this is truly anxiety what are your thoughts can this question also be anoynmous please?

Protected sex is safe as far as Hiv is concerned. However std's like genital herpes, hpv, syphilis can still be acquired despite protected sex. The reason being that a condom covers only a portion of shaft of penis and thus leaves a lot of neighbouring genital skin e.g base of shaft, pubic skin exposed for direct contact with genital skin & vaginal secretions of sexual partner during sexual intercourse.

Hpv and genital herpes can be acquired by direct skin to skin rubbing/ friction of unprotected genital skin i.e base of shaft and pubic skin, during sexual intercourse. The organism causing syphilis (treponeme) can even pass through intact skin if her vaginal secretions did came in contact with your unprotected skin at base of penis or pubic region during sexual intercourse because this area is usually not protected with a condom.

Rubbing with undergarments is not a risk factor for Hiv and other std's except hpv as most of these organisms do not survive outside body. Hpv remains a risk though.

I suggest you to get tested for genital herpes and syphilis at 4 weeks by ELISA for type 1&2 Hsv and RPR for syphilis, respectively.

Just a few follow up questions.

Say if there was a viral load in the underwear ? could this have a chance to go up my urethra and cause hIV infection? Can i move on and not worry about this episode at all regarding hiv as you said it doesn't live for long?
When I had touched the under garments I was not protected' just to confirm thanks sir.

I will follow up your advice and be tested for the other two STI's - what are there symptoms?

Hiv is not transmitted through clothes

Hi.

I don't think that rubbing your penis against her underwear poses any risk of Hiv because Hiv does not survive outside body for any length of time. Hiv is only transmitted by either vaginal, anal or oral sex with an infected sexual partner.

You don't have to worry about Hiv here. As far as other STDs are concerned you should get tested.

Genital herpes presents with multiple, painful, genital sores. The diagnosis is usually obvious clinically.

Hpv presents with warts, which are skin colored, asymptomatic papules.

Syphilis presents with an ulcer, which is usually solitary.
Syphilitic ulcer or chancre is painless (unlike in genital herpes which are painful), well defined, indurated i.e the base of hard and has a clear discharge at its floor. It is also associated with regional inguinal lymph node enlargement which is rubbery in texture and painless.
