1) can you palpate any hard nodule(Calcified Plaque)in the shaft of the penis?
2) Do you have painful erection ?
3) Are you able to penetrate vagina and perform enjoyable sex ?
I would suggest you to consult qualified Urologist for clinical examination and get Ultrasound Scanning of the Penis done to confirm the diagnosis .

I am attaching 3 new images so you can diagnose.
To answer your questions:
1) I cannot palpate any hard nodule. But there's a swollen vein on my penile shaft for last 2 years. When I pull the foreskin, I can see and feel a small lumpy cord. A doctor told me it was normal, so I didn't give it much thought. If I press that area, then it hurts a bit. I am attaching the image of the area along with the penile shaft with the vein. Kindly take a look.
2) I don't have painful erection, but I sometimes feel pain and ant-bite like feeling on my penis, especially when I masturbate a lot.
3) I don't have a sexual partner right now, so can't answer it. I can masturbate. Two weeks ago, I suddenly lost the ability to have erection for 2 days. A doctor told me it was because of psychological reasons. After two days, I regained the ability to have normal erection. Since then I've been worried about it.
For last two days, I am having soft erections. I a also feeling nervous and worrying about performance with a sexual partner.

I have been feeling some pain on the right side of my penis during erection. The pain is not constant, but sudden and throbbing in nature. It doesn't always accompany erection, but only sometimes. I feel pain moving my erect penis on the right side.

I have also been having sore ache and fullness in both scrotum after a trauma to left testicle. Could this be epididymitis? I have taken Hetrazan medicine for 21 days because a doctor diagnosed me for filaria. But the pain is now in pelvic area and sometimes in testicle. The pelvic area becomes very sore after I sit for long or walk for a long time. I also feel uncomfortable when I get erect.

Testes
Both testes are normal in size, shape and position. No focal SOL seen.
Right testis measures 36mm x 21mm
Left testis measures 36mm x 19mm
Epididymis
Head of the epididymis appears normal on both sides
Septum
Septum appears normal
Scrotal skin
Normal in thickness and texture
Cord
Cord is not thickened in right side
Left cord is thickened
Tunica vaginalis
No fluid is seen in tunica vaginalis on both sides
Impression: Left sided Funiculitis
Please analyze the report and suggest the treatment.

The doctor gave me 10 days course. Today is the sixth day and the tenderness and pain in my left testicle, groin and pelvic area has increased. I am really confused. I have pain in the underside of left testicle which was affected by the trauma. The pain is not severe but dull and it's a dense pain. It pains when I touch that area. Sitting aggravates pain and then it radiates to my left lower back and left thigh and left leg.

I have completed the two weeks course of antibiotics (doxycycline 100mg) and painkiller (combiflam). After that, my consulting urologists prescribed a course of Tab Calpol 650mg, twice a day for 7 days. Today is the third day I am on Calpol and the pain in the underside of the left testicle has considerably reduced. However, the swelling hasn't reduced yet. Both testicles feel a bit swollen when I touch them, even though the USG report shows they are in normal size.
My urologist said after taking Calpol for 7 days, I need to take Hetrazan tablet for 21 days because he thinks the infection is due to filaria. He also told me to wear tight underwear for 3 months.
Since my ultrasound report shows I have left sided Funiculitis, I am a bit confused as to whether the treatment is going on the right direction. Kindly give your opinion on this. Also when can I expect the pain and swelling to completely reduce?
