What Caues Breathing Difficulty When Suffering From Pulmonary Hypertension?

Question: Hello Doctor, my partner suffering with PAH is feeling breathless very often and also heart beat is very very fast while walking or even sitting idle. Can you please suggest what could be the reason for this sudden change in health conditions? Current medication is Bosentas 62.5 mg and Pulmo Pres. What needs to be done to get the condition to stability? Also what do these symptoms mean doctor?

Welcome To HCM. I am Dr Anshul Varshney.
I understand your concern.
A person who is suffering from PAH on already Tadafil and Bonsentas gets further symptomatic if PAH is increasing in severity.
I would like to know a few details to help you better:
1. What was the basic cause behind her PAH?
2. Any swelling on feet?
3. Any chest pain?
4. Any breathlessness on lying down or during sleeping?
5. Any blood in sputum?
6. What was the severity of her PAH? Mild / Moderate / Severe/ Very Severe
7. What is her resting pulse rate and Blood pressure?
8. Is she on any blood thinners / anti platelet drugs like Aspirin?

Hello Doctor,
Thank you for the reply, please find the response below.

1. Till now the basic cause is still not known doctor
2. Yes Doctor feet get swollen on and off.
3. Chest pain comes when the heart starts beating fast doctor and when breathless.
4. Yes Doctor, breathlessness is felt at intervals and also while resting
5. No blood in sputum
6. The severity has increased from Moderate to Severe now doctor.
7. Blood Pressure is at 104/70 doctor
8. No blood thinners as of now Doctor

Please help further in this regard doctor, worried very much. Please advice what all this means doctor.

Get a repeat ECHO.

Detailed Answer:
Dear Friend.
These signs simply means that her PAH severity is increasing.
I would advise a repeat ECHO.
She is already on a good treatment for PAH, but I suppose adding a diuretic would help her better.
So, get a repeat ECHO, Chest Xray.
Since the cause is not identified, I also advise CT Pulmonary Angiography to see for any obstruction or to rule out any chronic pulmonary thromboembolism.
You should take this matter seriously and get her a complete evaluation from an expert in your city.

A couple more questions doctor,

Any specific diuretic that can be prescribed doctor?

A CT Pulmonary Angiography was performed a few years ago, will attach the report as I get it from the repository doctor, will that be enough to decide if one more CT Pulmonary Angiography is needed doctor?

Is there a full time resolution for the ailment doctor? Also what are the limitations and what is the life expectancy doctor?

Another Angiography is not required.

Detailed Answer:
Dear Friend.
As far as diuretic is concerned, the decision is based on few parameters like Kidney function test, serum electrolytes, blood pressure. Torsemide is the one which I prefer in my patients who have all the above mentioned parameters normal.
Now if CT Angiography was performed and was normal , then there is no point in repeating it again.
There is no definitive cure for this, only symptomatic treatment is given.
As far as life expectancy is concerned, it is totally uncertain. Mortality usually occurs due to complications like Heart Attacks , Respiratiry failures , blood clots.
I would be in better position to answer, if I have your reports.
Review with all reports.

Get Autoimmune workup done.

I have reviwed the reports.
This is either Idiopathic Pulmonary Hypertension or Secondary to some disease like Autoimmune conditions.
I would strongly advise you to get Autoantibody Panel from a reliable laboratory.
She is not in front of me, so I can't examine her. But still I can say that cause should be ruled out as she is very young.
Get her above men test and write back with reports.
