                                            Question : Hello doctor, I have extreme back pain(middle of back) it radiates down both arms..especially when active. I've also noticed that my penis stings when urinating. I also need the toilet all the time. I've been checked for diabetes and my heart has been checked also(3 stress tests) Both were given the all clear. I recently had a flexible cystoscopy done as I previously had blood in my urine. I think the back problem begin after the cystoscopy. My arms and back ache. Is this serious?i forgot to say,I went to hospital and my blood work showed signs of infection. I was given antibiotics. I had my blood took yesterday as I had small bleeding in my stomach. The doctor said it was all ok. Please help doxtor. I've previously had urine infections, but nothing like this. I'm male 37years old. Slightly over weight(2stone) treated high blood pressure

You probably have a urinary infection, as a cystoscopy increases the chances. It would be advisable to have urine routine examination and culture. If it turns out to be a UTI, your doctor would prescribe the appropriate antibiotic for you.
Your long standing back ache can be arising from the spine. It may be good to have yourself tested for the same if not done already.

Unlikely to be from the heart as 2 TMTs were normal.
Detailed Answer:
Relax, it is very unlikely to be due to a heart problem. If you are so concerned, you could get a CT coronary angio done without catheterisation and I feel it would show any significant lesion.
Also I would not recommend Tramadol as it has got addiction potential and later you might find it difficult to come off it. You could use a combination of muscle relaxant and paracetamol for symptomatic relief.
Try to relax. A healthy wholesome food, losing weight if you are overweight, moderate exercise at least 30 minutes a day, stress relieving measures like yoga, meditation or relaxation exercises would help.

                                    Thanks doctor, I asked my doctor previously for a coronary angio. He said that because I've had 3 clear stress tests, he is happy to rule out heart problems or disease(at this stage) so I will try to relax.
