Question: My tmt,eco and ecg is negative . I am a diabetic and feel chest pain daily. My pschictirist says it is due to anxiety. He has given me Nusam 400 twice and Mirtaz 30 at night . Taking this from 10 days . Before that Mirtaz 15 . What should I do ? Is my symptoms are due to costiocondritis . I feel chest pain while walking as well at left side and other symptoms of heart disease . I am a hypochondriac since my childhood and having chest pain . I performed TMt till 10.5 mets. I can't find courage to go for ct angiography. But no Doctor has suggested me for that . I have taken Ssri but it doesn't suit me . I have IBS issues as well . Always worry for different diseases. What I think, I feel the symptoms of that disease. I have migraine issues frequently. I have diabetes since 5 years. I am 42 years old and start taking Atorvastetin when diabetes started. I also take clopilet 75 by insisting my Doctor. My total cholesterol is 160. Pls advice . I can go for Ct angiogram but after some days other health worry will start . I am taking Co Enzyme Q 10 and Omega 3 as well . Pls advice . Should I stop worrying about heart disease .

This is Dr.Riyanka and I am glad to be helping you with your query.
Since you are already on treatment for anxiety with Nusam and Mirtaz, and your TMT, ECG and echocardiography are within normal limits, a cardiac cause of your problem is less likely and your psychiatrist is possibly right.

Mirtaz (Mirtazapine) and Nusam (S-adenosylmethionine) are both given for anxiety disorders. SInce your Mirtax has been recently increased, you might take about 15 days to adjust to the higher dose and your symptoms to be controlled.

Chest pain that is sharp and present most of the time including while walking can be costochondritis which is a benign inflammation in the front of ribs. This is usually relieved by pain killers and can stay unchanged for weeks to months.

CT angiography is not compulsory but you can go ahead for peace of mind. It is a completely non invasive test and take less than 5 minutes for the actual procedure. This will show any possible blocks in your coronary arteries and that is very unlikely if other tests are within normal limits. You have to get a basic kidney function test and inform of any drug allergy and other medications at the time of scan.

In my opinion please stop worrying about heart diseases and allow your anti anxiety medication some time to produce the desired effect and control your symptoms.

Meanwhile you can try doing meditation for 30 minutes each day for relaxation.
