Question: I feel dizzy and get a smell of acetate in my nose at times. When resting or driving a car. I am pre diabetic, and obese. I take Hydralzine . and Diavan and Sotolol. Fasting blood sugar 125 range, BP 155?74 Heart rate 69 with a pacemaker. Woke up in the night with dizziness and smell, vomitted flemm, temperature 99.2. Light headness is sporatic

I have gone through you query and I understand your concerns.

Basing on your medical history and on your symptoms I can say that you suffer from Transient Cerebral Ischaemia(TCI). TCI is a complication of Sinus arrhythemia and it is caused by blood clots.

Dizziness and vomiting is caused by Transient Cerebral Ischaemia.
Your symptoms are aggravated by Diabetes mellitus. Acetate smell is caused by high glucose level.

To prevent transient ischemic attack follow these advises:
- You shouldn't eat foods that contain Vitamin K, such as spinach, broccoli, XXXXXXX etc, because foods that contain K vitamin make your blood clot.
- Keep your glucose level under control by eliminating carbohydrates from your diet.

You need an INR test(international normalized ratio test) to check blood coagulation. The result of INR test will determine if you need anticoagulant medicines.

You also need HbA1c test to check glucose level. This test is more accurate then glucose test because it measure the level of glycated hemoglobin.

Consult your endocrinologist and your cardiologist, to discuss about the treatment you need.

Thank you, you are the first professional to give me an answer. I walk 2 miles a day and garden.I do take Xarelto 20mgfor 1 year for clot prevention.I also take Crestor chlorthalidone25, Potassium time release,Diovan 320, and Ranexa 1000mg x 2, and sertraline 50mg as an antidepressant.
The dizzy spells have been less frequent but maybe a stomach virus going around aggravated it. It seems short walks and driving have been trigger them

I saw your treatment very carefully and I can say that it is a correct treatment.

But Chlorthalidone 25 mg and Ranexa 1000 mg are causing your symptoms.
Symptoms that you are experiencing are side effects of these medicines.
Chlorthadine and Ranexa cause dizziness while standing. This is the reason why short walks aggravate dizziness.
Stomachache is caused by these two medicines, too.

You should avoid standing for a long time, in order to relieve your symptoms.

If symptoms that you are experiencing become severe you should consult your cardiologist to discuss about the treatment.
