What Causes Dizziness And Troubled Vision?

Question: Dear Doctor,
I hope you are really well ?
My name isXXXX:)
I am 40 years old and of pretty good health (normally). I exercise ALOT, am 52 kgs. Drink loads of water, eat really well. Have MAXIMUM 2 glasses wine a night weekdays- and weekends probably a bottle in total.

I am praying for some kind of help - my doctor passed away afew years ago and the new doctor is HOPELESS :(

For about 3 years now i have been experiencing alot of dizziness, troulbe focussing - like tunnel vision or something etc. I have literally just put up with it. Always keep my sugars up as about 8 years ago i had low blood sugar however this seems to not show up anymore in blood tests.

Lately tho - its just unbearable. I have been having alot of trouble focussing driving - and a month ago i almost literally blacked out (well not blacked out - but more so vision went all tunnel weird and my heart raced and hands went all sweaty and shakey etc. I had to pull over as i couldnt barely concentrate on driving and i havent driven since. I had a CT scan on my brain whic was clear ?
I have had sooo many blood tests - NOTHING !!!
Then about 6 weeks ago i demanded a urine test and explained alot of pain in my left lower side back Anyway - it came back as ECOLI. He gave me antibiotics - 2 lots. But unfortuantely i am no better & the pain in my lower left side /back. Still really dizzy, ridiculously tired, still cant focus in the car its even a struggle crossing the road (checking if there are cars etc). Sometimes feel like ive lost feeling in my legs etc. Sometimes feel nauseas. I get pins and needles all the time in my hands at night and feet and muscle are sore alot. I have had full eye tests and my eyes are perfect ???

I was adopted and the only info i have on my natural parents is "unspecified kidney problems" ?

Im reallly worried and am struggling to get through each day & just want to be back to my normal fit energetic self :(

Any suggestions / diagnosis / what to test next - etc would be so greatly appreicated ?

Hi,

I had gone through your query and understood your concerns. I am very sorry for the condition you are facing.

After careful analysis of your symptoms, I am not sure if I can give you a definitive diagnosis. However I would consider the possibility of excessive insulin producing condition called INSULINOMA in this context. The family history what you gave is very significant. In a condition called MEN syndrome there will be excessive insulin production along with renal stones. The pain in your lower back is also significant it might be due to renal stones ,which is adding strength to this possibility.
When this insulin secretion increases it will cause imbalance in potassium and cause pain in your muscles.

You should discuss about this possibility with your treating doctors. If they agree, you will need to under go battery of tests including:
1. Abdominal CT
2. Insulin assay during 12 hrs fasting or when you get hypoglycemia
3. C peptide assay
4. Serum calcium
5. Parathyroid hormone assay

I suppose I have given the desired response if you have any other query please write back.

Can i trouble i eith afew follow up questions ?
So would the dizzyness (trouble focussing to drive / heart racing etc) be in line with your diagnosis ? Is the renal kidney thing part of the sympons of your diagnosis or is the sore back kidney thing unrelated and seperste ? The pain is agonising !!Many thx in advance XXXX

Hypoglycemia symptoms

Dear,

Hypoglycemia will usually present with neuroglycopenic symptoms which includes
1.increased heart rate
2.sweating
3.confusion,lack of judgement
4.tremor and shaking in limbs
5.blurred vision

Further in MEN 1 syndrome which is also called as Wermer's syndrome, there is asymptomatic hypercalcemia where the calcium levels is found to be high leading to renal stones (nephrolithiasis or nephrocalcinosis).

Both hypoglycemia and renal stones are related the possibility.

One of my patients, who got family history of this problem had undergone surgery with a oncologist and now are leading comfortable life. Honestly, I don't diagnose you with this condition; but it's definitely worth evaluation in these lines since there are many matching symptoms. Please discuss with your treating doctor.

Dear doctor, thanks for advice so far. so i had ct scans on my kidneys and abdomin yesrterday and eferything came back ok but showed a 2.5mm cyst in ovary?? Also another urine test came back still showing uti - with my leuctcytes level at 100??? He had given me a 3rd dose of antibiotics - this time stronger ones for the ecoli - i just wondered if all my asymptons could just be from the uti/ ecoli infection ? And if u would suggest any oher tests ?Im guessing i could have had his infection for a longggg time. Thanks again XXXXXXX xox

I am very happy to know that CT scan report is normal.
I would like to know about insulin assay and C-peptide assay also.
In my opinion during excessive infection there will be up regulation of acute phase proteins which might be the root cause for all this symptoms.
Regarding urinary tract infection i suggest you to go for culture and sensitivity testing.
At some times this bacteria E coli will be resistant to conventional antibiotics.
By testing culture and sensitivity we can choose right antibiotic for bacteria in you.
i suggest you to drink ample of water and cranberry juice which will help in removing bacteria quickly.
i wish you for speedy recovery from infection

Ovarian Cysts may not cause dizziness

Hi XXXXXXX Hope you are feeling better after starting new antibiotics.

Usually this ovarian cyst may give a dull abdominal pain and in worst case scenario they may cause acute abdomen due to twisting also called torsion of cyst.

Dizziness can be due to low haemoglobin or some times ear related problem called vertigo. Your local doctor will know about these conditions.

Daily do exercise, eat balance diet and enjoy good health
