Based on your query, my opinion is as follows:
1. Filtration rate or glomerular filtration rate is a test for renal function. It is the ability of the kidney to filter blood and dispose of the waste products.
2. Normally it is more than 90. At present, here it is reduced from 48 to 40, which indicates moderate to severe loss of kidney function.
3. As the kidney is filtering less, if the volume of fluid were to be increased, the total amount of filtered volume will increase. Hence, you doctor has advised you to take more water. This will reduce the amount of waste products, which can be retained in the body with lesser fluid filtration.
4. Avoid any medications, which can cause further renal damage. Also avoid, excessive salt intake or dehydration. Any chronic diseases like diabetes or hypertension, needs to be under strict control.
5. Any test, which will impact kidney, should be strictly avoided.
Double the volume of water, you usually take. It should help. Avoid any diuretics at present, as they can increase load on kidney. If any swelling in face is seen or urine output is reduced, do meet you doctor immediately.
