Question: My 25 yr old grand dtr. had back surgery for a disk through the front abdomen 5 days ago and is in severe cryi8ng pain and not eating , low grade fever,,99.8 but has been pracitcally screeming all day. Last few hours she is violently throwing up green foam emesis several time which tears at her stitches in the front. Crying all the time. Oxycondone stopped yesterdday and dr. won't give her more but she reacts to ibuprofen and tylenol. and having surgery probably shouldn't take ASA. But they gave her nothing to take as of yesterday. The vomit and the diarrhea and both green which I believe is staph. and has distinctive odor. I think she she go to er. do you agree?

Detailed Answer:
Peritonitis is an inflammation of the peritoneum, the thin tissue that lines the inner wall of the abdomen and covers most of the abdominal organs. Peritonitis may be localized or generalized, and may result from infection.

This is a serious condition that needs immediate medical attention.Fever is suggestive of active infection most likely bacterial in her case.symptoms include pain in the abdomen, nausea and vomiting,pain fever cramps etc.

Take her to emergency room asap.
Swab from her stiches and stool routine test should be done to find out the bacteria invoved.

As she is a post operative case most likely involved bacteria can be staphylococcus or MSRA.

This should not be considered lightly.
Injectible therapy with Tazobactam and Piperacillin 4.45 gram thrice daily should be started.
Linozelid should be considered from MRSA.


Initially take her to Emergency room.

I take it she should stay in hospital a bit. Is she infectious if staph or mrsa and need to be in isolation. How does the bacteria get into the stomach and colon to cause green pus if the germ is on the outside of the organs in the peritoneal area? Also, is she at risk for septecemia or blood infection which I know is very serious.

Don't waste time of patient and reach emergency room quickly.

Hi again,
Its not necessary that she is having perotinitis.I am just trying to give you a differential diagnosis.
Treatment is same whether its perotinitis or infection of any other organ by bacteria or Septicemia( infection of the blood).
All these terms are interconnected whether perotinitis or septicemia etc.
See if peritoneal cavity is infected or site of stiches are infected bacteria would reach blood stream.
Pus is suggestive of fact that there is severe bacterial infection.
Bacterial infection is the initial phase if not treated properly it progresses sepsis and can even lead to septicemia.
