What Causes Hard Swollen Painful Testicles?

Question: My 23 yr old son says his testicles are somewhat swollen and hard. This has been going on for a while. He defiantly notices a difference. Testes are somewhat hard to the touch. He says the testes are very sensitive. If he accidentally rubs up against something it hurts. He is a waiter and does heavy lifting of food trays. I'm not sure what to tell him about this situation. Any suggestions??? Thank you..

His symptoms can be caused by testicle inflammation, testicle cancer, inguinal hernia.
If he has also fever and frequent urinating, infection is most probable.
If he is able to palpate a hard lump, cancer is possible.
Inguinal hernia can give more intense pain when lifting or coughing.

I suggest him to do not wait with it any longer. He should consult urologist and have testis ultrasound at first.
