Question : Hi,
I'm worried about my father in law. I just spoke with him and he has had a high fever of 104.6 for 2 days, skin feels like it is crawling and trouble urinating. Trouble meaning he feels like he has to go but then only dripples out. No Hx of prostate issues. No other symptoms before.

Hello,

the most likely scenario is a urinary tract infection either a pyelonephritis or a prostatitis. Taking his urination problems into account the latter sounds more likely. Immediate medical attention is required because both are serious infections and may even cause death if left untreated or if treatment is delayed.

A urinalysis and urine culture would be important tests to do before starting antibiotics but they shouldn't delay treatment if they're not available.
