Question : When I walk and increase my heart rate, I get pain under my left collarbone, that radiates out about 3 inches in all directions. If I take a deep breath and hold it, the pain subsides. Otherwise the pain stops on its own after a few seconds after I stop exerting myself. If I start walking again, the pain comes back. I went to urgent care, a Cardiologist, and a screening clinic, and have had 3 ekgs and blood work and everything seems to be fine. Only problem was my hdl level was just below normal. My ldl level was 36. Also Aortic and abdominal ultrasound scans found no problems. I am 61, weigh 194, (got down to 184 with all vegan diet for 1 month), don't eat sugar, :/ 120/80 average blood press., eat lots of fruit and vegis, average to low meat consumption. Rarely eat red meat, mainly chicken, all baked, nothing fried.

Detailed Answer:
Hello!

Welcome and thank you for asking on HCM!

Your symptoms could be related to a musculo-skeletal pain or gastro-esophageal reflux.

The fact that the pain subsides by taking a deep breath, is not typical of a cardiac disorder. It is more indicative of a musculo-skeletal pain. The fact that your repeated ECG and blood work result normal, is another reassuring argument.

But, considering your age, I would recommend performing an exercise stress test too, to exclude possible coronary artery disease, although your symptoms are not typical of this disorder.

I would also suggest making some trials:

1- Trying taking ibuprofen. If it helps relieve the pain, it would be indicative of a musculoskeletal disorder.
2- Trying taking Omeprazole. If it helps relieve the pain, it would be indicative of gastro-esophageal reflux.

A chest X ray is needed too. If Omeprazole helps relieve the pain, I would recommend performing a fibrogastroscopy.

You should discuss with your doctor on the above issues.

Hope to have been helpful!

Kind regards,

Dr. Iliri
