What Causes Inflammation And Red Rashes On Penis And Itchy, Inflamed Scalp?

default
Posted on Mon, 29 Dec 2014
Question: 7 months ago my symptoms started, red rash on penis which was inflamed - this still remains, small lesion on lip lasting 1 week, 2 weeks later a blustery rash lasting one week on torso. 2 weeks later, very itchy scalp which felt hot and inflamed, 1 month later hairloss disperse which has continued since. A few Sporadic blister like spots randomly appear anywhere on scalp. Head was itchy for one month only.
Scalp biopsy taken in July, inconclusive, but possible lpp, treatment ( malaria tabs, doxy, cellcept) hasn't responded except for steroid injections which are temporary (3/4 week relief from abnormal hair loss).

7 months on, 10+ derm visits later and still unsure what is causing this and hair thinning is worsening.

Any ideas please???

Brief Answer:
Lichen Planus Pigmentosus

Detailed Answer:
Hi.

I have received scalp photos but it is not very clear. I have not received penis photos
Considering that you have been given a possibility of Lichen Planus Pigmentosus(lpp) and also going through the treatment history and response to steroids, it is imperative that you continue with the treatment and follow up regularly with your treating doctor.
LPP is a cause of cicatricial alopecia i.e irreversible hair loss. The patches of LPP may gradually extend to involve wider scalp areas and if allowed to progress unchecked it may leave behind parchment like scarred patches of irreversible hair loss.
Treatment modalities of LPP are oral steroids, either daily or pulse dosing. Steroids are an essential part of treatment because they arrest progression and may also reverse early damage. I usually start my patients on twice weekly doses of steroids (every saturday and sunday) and continue it for 4-6 months till the disease activity subsides,
Antimalarials are also useful in LPP.
Usually this disease is self limiting and regular treatment is to be continued till the disease activity subsides, usually 4-6 months.

Regards
