Question : What can cause an intermittent fever (usually normal upon awaking and high by about 3 degrees at bedtime) plus intermittent nausea? Type 1 diabetic for 38 years, definitely experiencing diabetic gastroparesis, being treated with Xyfaxan and most recently taking Bactrim. No change in symptoms, in fact may be getting worse.

I have gone through your query.
How long you have fever, is your blood work done.

I would have asked for complete blood picture with peripheral smear for malarial parasite, as v you have mentioned about the evening rise in temperature I would have asked for sputum examination for acid fast bacilli. Based on the result I would have considered the management.
